Description:		Kern County Public Health is seeking a qualified organization to provide Hepatitis C Virus (HCV) navigation and linkage to care for people including education, enrollment in health insurance, scheduling appointments, and transportation for medical appointments with provision of HCV care coordination and assurance of treatment including support in obtaining medication from pharmacy, evaluating and overcoming compliance issues, monitoring side effects, and patient follow up.
