Basedon an assessment of existing resources and extensive input, the County is exploring the option of contracting withCommunity Health Centers (CHCs) to provide primary care medical services co-located with social services and access toprescription medication to address anticipated gaps in services due to barriers created by shifts in federal policy. In addition,the CHC would deliver primary care services through telehealth modalities, including video and telephone visits, to supportaccessible and timely care and through Mobile Medical Units (MMUs), including routine and preventive care provided atscheduled community locations, including community health fairs.This RFI is issued solely for information and planning purposes – it does not constitute a solicitation/Request forProposal/etc. or a promise to issue a solicitation in the future. This request for information does not commit the County tocontract for any supply or service whatsoever. Respondents are advised that the County will not pay for any information oradministrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at theinterested party’s expense. Not responding to this RFI does not preclude participation in any future solicitations.
