SMMC operates outpatient clinics throughout San Mateo County, an acute-care hospital, and long-term care facilities in San Mateo and Burlingame. San Mateo Medical Center serves the health care needs of all residents of San Mateo County, with an emphasis on education and prevention, and without regard for ability to pay.

The County of San Mateo is seeking proposals for Value-based Care Innovation Clinical Partner . The estimated Budget is $255,000 .

Background

The Innovation Center at San Mateo Medical Center is a unit focused on value-based care and participatory innovations with our patient community. The Innovation Center is seeking a value-based care innovation clinical partner for participating in hands-on implementation of its projects and in the development of the innovation center from a clinical/medical lens.
