By said Service whichincludes all medical components for systems of care, supported by public policy that integrate orinterface with the EMS system. INTRODUCTIONShelby County Government, (the “County”), is soliciting proposals from interested andqualified Vendors to provide provide the services of an Emergency Medical Services Directorfor the Shelby County Fire Department. This Request for Proposal (“RFP”) is being releasedto invite interested and qualified Vendors to prepare and submit proposals in accordance withinstructions provided where the successful proposer(s) will be selected and invited to enterinto a contractual relationship with Shelby County for the Goods and/or Services outlined inthis RFP, and per proposal(s) received.In this RFP, the terms Proposer, Vendor, Bidder, Consultant, Contractor, and Provider areused interchangeably unless the context indicates otherwise.All specifications are part of this bid and general terms and conditions (as outlined in theTerms of this bid). They shall become a part of and be included in any contract/purchaseorder/systems contract resulting from any award.These specifications are not intentionally written to favor any one manufacturer or serviceprovider, and are only for the purpose of indicating generally the type of work required of saidbid. Any responsible Bidder who considers these specifications to be of a non-competitivenature should immediately contact the Administrator of Purchasing.II. MINIMUM PROPOSER REQUIREMENTSIn order to be considered as “responsive”, all Proposers must answer and providejustifications for ALL below minimal requirements.The information requested below to be answered directly in the scorecard Excelfile by all Vendors, and submitted part of your proposal (both Printed andDigital formats).1. Provide all appropriate Licenses and Certifications required in the State of Tennesseeto provide the goods and/or perform the Services required. Provide a copy of yourcurrent Shelby County Business License (if business is located in Shelby County,TN).2. Must provide active Equal Opportunity Compliance (EOC) number(s) and Vendornumber, or your applications are “in” the EOC system and the Purchasing system forprocessing (refer to details outlined below) – please list all your Shelby County EOCactive numbers.3. Adherence to all provisions of Title VI requirements – please attest, and provideproof/documentation if necessary.4. Independent Vendors (sole proprietors) must adhere to State of Tennessee PublicChapter No. 436, known as the “Tennessee Lawful Employment Act” (effective dateof 01/01/12) – please provide proof and documentation of employment eligibility4(driver’s license…); Otherwise mention you are NOT an Independent Vendors (soleproprietors).5. FORMS - EOC Compliance Form A must be completed with active Shelby CountyGovernment LOSB vendors listed, signed and included with your proposal, as detailedin this document. FORM B to be completed/signed, with applicable backup, only IFthe goal is not met on Form A.6. FORM – Drug-Free Workplace Affidavit must be completed, signed and notarizedwith your bid/proposal – even if less than 5 employees.7. Must attest to a minimum of _ten (10) years of experience providing the goods and/orperforming the services described in this bid.Shelby County EOC and Vendor numbersAs a part of doing business with Shelby County Government (SCG), each individual, companyor organization is required to have active EOC and Vendor numbers before starting to providegoods/services to SCG.• To obtain a SCG Equal Opportunity Compliance (EOC) number (as well asLOSB number, if applicable) and the SCG Vendor number, please submit anonline application, as described below.
