Parking Lot1. The contractor shall apply de-icing material to the main administrative officeparking area which includes the two driveways. One leads out to highway 5 andone leads to 18th street.2. The contractor shall apply de-icing material to parking lot when outside airtemperature is below 30 degrees and snow or rainfall has occurred or unlessotherwise directed by the Contract Officer Representative (C.O.R.).3. All de-icing shall be done before 6:00 a.m. Monday through Friday. No de-icingshall be done between the hours of 6:00 a.m. and 5:00 p.m. Monday throughFriday unless otherwise directed by the C.O.R. It will not be necessary to de-icebefore weekend days or state observed holidays (Veteran’s Day, ThanksgivingDay, Christmas Day, New Year’s Day, Martin Luther King Day, and President’sDay). Typically, de-icing should only be completed once in a twenty-four hourperiod. In rare instances, the contractor may be requested to de-ice more than oneoccasion within a twenty-four hour period.B. The contractor and the C.O.R. will conduct a pre-work meeting before the start of thiscontract to discuss de-icing patterns, and special considerations to protect thefacility’s lawn, landscaping, and buildings.IDL RFQ 26-8302Page 4 of 18DE-ICING SERVICESTJ SUPERVISORY AREA OFFICEIDAHO DEPARTMENT OF LANDSSTANDARD INFORMATIONADDENDAIt will be the respondent’s responsibility to check for any addenda prior to submitting a quotation. In the event itbecomes necessary to revise any part of the solicitation documents, addenda will be made available.Information given to a respondent will be available to all other respondents if such information is necessary forpurposes of submitting a quotation or if failure to give such information would be prejudicial to uninformedrespondents.BURDEN OF PROOFANY VARIATIONS of brand names or deviations from the specifications MUST BE CLEARLY STATED. Itshall be the responsibility and burden of the submitting vendor to furnish the State WITH ITS ORIGINALSUBMISSION sufficient data to determine if the goods or services offered conform to the specifications.ORAL INFORMATIONThe State will not be responsible for any verbal or oral information regarding a quote.DISQUALIFICATION AND AWARD INFORMATIONThe state reserves the right to make reasonable inquiry to determine the responsibility of a contractor.Such requests may include but not be limited to financial statements, credit ratings, statements ofexperience and past performance, references, etc. Successful contractors must show to the satisfaction ofthe Idaho Department of Lands that they have sufficient equipment and work crews to complete the workcontracted by the time specified. The unreasonable failure of a contractor to promptly supply information inconnection with such a request is reason for disqualification. Except as otherwise provided by law,information furnished by the contractor pursuant to this provision may not be disclosed outside the IdahoDepartment of Lands without prior written consent of the Contractor. Disqualification of a high rankingcontractor may be pursued when their reputation, experience or references are such as to create a doubtabout satisfactory job completion or if the price quotes are considerably below Department estimates andthe other quotes. The purchasing agent will contact the contractor and request that they disqualifythemselves by withdrawing in writing. If the contractor refuses to withdraw, the purchasing agent may notifythe contractor in writing or email that the Department will not offer the contractor a contract and proceed withan award to the next responsible contractor.PARTNERSHIPSContractors responding as partners must furnish the Idaho Department of Lands the name of thepartnership, names of the partners, and the partnership's federal taxpayer ID number. All payments will bemade to the partnership.INTERNAL REVENUE SERVICES REPORTING REQUIREMENTIRS rules and regulations require employers to submit a miscellaneous income form (IRS form 1099) for allcontractual persons who receive $600 or more in a calendar year. Incorporated firms are exempt from thisreporting requirement. The contractor's taxpayer identification number (Social Security or employer number)must be listed on the signature page of the contract.PUBLIC RECORDSThe Idaho Public Records Law, Idaho Code Sections 74-101 through 74-126, allows the open inspectionand copying of public records. Public records include any writing containing information relating to theconduct or administration of the public's business prepared, owned, used, or retained by a state or localagency regardless of the physical form or character.
