Summary **TERM CONTRACT REQUEST** *Annual usage estimate 100 rolls (25,000 decals)* Initial order - OSHP K-9 Decals. 27 originals printed one side, full color, 2.5" round finish size with full bleed and excess weeded away. White 2.4 mil polypropylene, over-laminated, pressure sensitive, permanent adhesive. 250/roll with 1 roll for each separate K-9. Backing should be either perforated every 3" so that they decals can be torn off and handed out. Future Orders - May be ordered as single roll at a time. PDF quote required! STANDARD TERMS AND CONDITIONS: The State of Ohio Standard Terms and Conditions (revised 03/31/25, attached) apply to this solicitation and will govern any resulting Contract. EVALUATION: Bids will be evaluated in accordance with ORC 125.07. In addition, the state will multiply the bid prices by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items. CONTRACT AWARD: The contract will be awarded to the lowest responsive and responsible bidder by low lot total. The low lot total will be determined by multiplying the unit prices by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items. Failure to bid on all items may result in the bidder being deemed not responsive.
