Middletown Area School District55 West Water Street, Middletown, PA 17057REQUEST FOR PROPOSALS (RFP)Part-Time Physical Therapist ServicesRFP Issue Date: Monday, May 18, 2026Proposal Due Date: Friday, June 5, 2026Anticipated Start Date: Beginning with the dates identified on the attached 2026-27 AcademicCalendar or earlier, as requested, for needs during the summer of 20261. PURPOSEThe Middletown Area School District ("District") is seeking proposals from qualified individualsor businesses to provide part-time physical therapy services for eligible students in accordancewith IEPs, Section 504 Plans, and applicable regulations.2. BACKGROUNDThe District serves students in grades K–12 and is committed to providing high-quality specialeducation services. Beginning with the 2026-2027 school year, all district schools are scheduledto be located on one campus.3. SCOPE OF SERVICES- Provide evaluations and re-evaluations, including early intervention transition that could occurduring the summer- Develop and implement therapy plans aligned with IEP goals- Deliver direct and consultative services- Obtain and/or maintain compliant documentation, including, but not limited to, progressmonitoring records, School-Based medical access billing, physician authorizations.- Attend IEP meetings, in-person or remote as authorized by Director of Special Education- Collaborate with staff, families and stakeholders- Recommend equipment/modifications and obtain equipment at the lowest possible price incoordination with the Director of Special Education or designeeService Locations:Middletown Area High School, 1155 N Union Street, Middletown, PA 17057Middletown Area Middle School. 215 Blue Raider Lane, Middletown, PA 17057Reid Elementary School, 115 Blue Raider Lane, Middletown, PA 17057Middletown Area Primary School, 315 Blue Raider Lane, Middletown, PA 17057Level of Effort: Part-time, approximately 25-30 hours per week during the academic year. Anyhours needed beyond 25-30 hours per week must be authorized by the Director of SpecialEducation in advance.4. QUALIFICATIONS- PA Licensed Physical Therapist- School-based experience in a low incidence (multiple disabilities, life skills, autism) classroompreferred- Knowledge of IDEA and Section 504 of the Rehabilitation act of 1973- Completion of child abuse training- Required clearances (Act 34, 151, 114, 168)- Proof of liability insurance5. CONTRACT TERM –The initial contract term will be for one (1), three (3) or five (5) years beginning July 1, 2026.Contract can be cancelled with a 30-day written notice by the District prior to the expiration ofthe contract if the District is not satisfied with the services being provided or due to negligenceof the provider.6. PROPOSAL REQUIREMENTSCover Letter, Qualifications of Vendor and Proposed Therapist (including licenses andcertifications), Cost Proposal, References (school districts preferred), Vendor Response to RFPQuestions, Form W-9, Completed Agreement- with the exception of the term to be decided bythe District7. TERMS AND CONDITIONSDistrict reserves rights to reject proposals and waive irregularities. District may award contractsto multiple vendors in order to meet the service demand of the District.10.
