INTRODUCTIONSan Lorenzo Valley Water District (SLVWD or District) is a rural/urban water supplier toapproximately 23,700 customers in Santa Cruz County, California. The District’s legal boundariesencompass approximately 62 square miles, including a service area of approximately 26 square milesstretching from Scotts Valley to Boulder Creek.The District’s scheduled Capital Improvement Projects include new underground pipelines, new waterstorage tanks, soldier pile retaining walls with horizontal tie-backs, rehabilitation of existing waterstorage tanks, new masonry pump stations, rehabilitation of existing pump stations & pipelineinstallations on new bridges by Caltrans & the County of Santa Cruz.II. PROJECT SCOPE OF SERVICESThe San Lorenzo Valley Water District is soliciting proposals to provide On-Call Special Inspection andTesting services for a period of 3 years. Qualified firms, at a minimum, are expected to be able to providespecialty inspection/testing in the disciplines below. An approximate yearly frequency is provided, butplease note this value is an estimate for reference purposes and does not constitute a minimum ormaximum frequency. The selected firm shall be expected to promptly provide services within 2 businessdays of receiving notice from the District, though advanced notice will be provided when available.- Compaction Testing (trenches, roads, foundations)o 50 tests/year- Concrete Strength Testing (cylinder samples)o 5 tests/year- Welding Inspection (continuous and periodic)o 4 tests/year- Pervious Concrete Testing (infiltration rate)o 1 test/year- Ground Anchor Pull Testing (for certain retaining walls)o 2 tests/year- Pre-Cast Lagging (for certain retaining walls)o 1 test/year- Masonry Inspection (for CMU buildings, namely pump stations)o 3 tests/year- Epoxy Anchor Inspectiono 2 tests/yearThe selected consultant will be expected to review project plans to provide the expected type andamount of required tests and inspections. Such estimates shall be the basis for a project specific Not-To-Exceed (NTE) value. All work shall be billed hourly to each specific project.Project plans for three upcoming 2026 construction projects (Highland Tank, Redwood Park Tank, andEcho Tank) are included as appendices to this Request for Qualifications. Consultants shall review andprovide an inspection list with an estimated Not-To-Exceed value for each project. (See ProposalRequirements Item 6)Deliverables may include some or all of the following, depending on the project:1. Testing and Inspection Reports2. Site Visits3. Engineering Plan Review4. Special Inspection Final Letter / Statement of Special InspectionThe Consultant shall provide internal quality control and quality assurance procedures. Anysubconsultants employed by the Consultant shall be managed by the Consultant.III. SPECIAL CONDITIONSAll SLVWD projects must comply with prevailing wage (per California DIR requirements).Work hours on SLVWD projects are limited to 8AM-5PM.IV. PROPOSAL REQUIREMENTSStatement of Qualifications must be clear, succinct, and not exceed 17 pages, excluding coverletter and resumes, on 8.5 x 11-inch paper of no less than 12-point font. Responses must followthe format outlined herein. SLVWD may reject as non-responsive, at its sole discretion, anyproposal or part thereof that is incomplete, inadequate in its response, or departs in anysubstantive way from the required format. Proposal responses shall be organized in the followingmanner.1. Cover Letter/Letter of Intent (1-page maximum)2. Scope of Work, Project Understanding, and Schedule (2-page maximum) A statement concerning the ability of the firm to perform the services within a timelymanner. A detailed explanation of how your firm would approach the work. It may include adescription of tasks, products, milestones, and timetables. Deliverable descriptions shouldfully discuss the steps to be followed in carrying out the work. Sufficient detail should bepresented to show a clear understanding of the work and the proposed approach, includingfor the three upcoming projects.3. Identification of Prime Consultant (2-page maximum) Legal name and address of the company. Legal form of company (partnership, corporation). If company is wholly owned subsidiary of a "parent company," identify the "parentcompany." Name, title, address and telephone number of person to contact concerning the Submittal. Project team and the discipline/job title of each team member. (Resumes of project staff willnot be considered as part of the total page count maximum) Provide a general description of your firm’s background and project qualifications, includingyears of business, any past bankruptcy filings, and identify any contract or subcontract by thefirm which has been terminated, in default, or had claims made against it that resulted inlitigation or arbitration in the last five years.4. Experience with Similar Kinds of Work (5-page maximum) Previous relevant project experience, including:i) Lead agency, contact name, and phone numberii) Project size and descriptioniii) Project budget and total value of completed projectiv) Budgeted project schedule and total time to completion5. Identification of Sub Consultants, if any (2-page maximum) Legal name and address of the company. Name, title, address and telephone number of prime contact. Provide a general description of subconsultant’s background and project. qualifications,including years of business, any past bankruptcy filings, and identify any agreement orcontract by the firm which has been terminated, in default, or had claims made against it thatresulted in litigation or arbitration in the last five years.6. Project-Specific Inspection/Testing and NTE Estimates (3-page maximum) For each of the three attached projects, Consultant shall provide:i) A list of required inspections/testing to be performedii) An estimated NTE value encompassing all anticipated inspections/testing7. Compensation Schedule (2-page maximum) Consultant shall identify, for each testing/inspection discipline, an hourly rate for all staff(including sub-consultants).Note that the proposal and compensation schedule are anticipated to become an attachmentto the contract between the Consultant and the District.V. CONSULTANT SELECTIONFailure to meet the requirements of this RFP will be cause for eliminating the applicant fromfurther consideration. Based on the District’s evaluation, the firms that meet the requirements ofthis RFP will be ranked. The following weighted criteria will be used to evaluate the Proposalsprovided in response to this request:EVALUATION CRITERIAUnderstanding of Scope of Work RequestedExperience with Similar Kinds of WorkProposed Compensation Schedule and Project EstimatesQuality of Staff Assigned to ProjectFirm’s Local ExperienceTOTAL:Weight15%20%50%10%5%100%VI. SELECTION PROCESSIt is anticipated that the District will enter into an agreement with the highest-ranking firm.However, the District reserves the right to consider other factors such as overall cost and mayselect any qualified applicant, regardless of the assigned rank. The District will enter intonegotiations with the selected firm. If the District is not able to negotiate an agreement that isfair and reasonable in the District’s sole discretion, it reserves the right to select an alternatefirm. The selected firm must enter into a written agreement using the District’s standardProfessional Services Agreement which is found as Appendix. Negotiations will cover: scope ofwork, agreement terms and conditions, office arrangements, attendance requirements and theproposed fee schedule.VII.
