(Louisiana R.S. 38:2212-C-2).3.4.4It shall be the Quoter’s responsibility to make inquiry as to the Addenda issued.4.0CHANGE ORDERS4.1All Public Works’ contracts shall contain provisions authorizing the issuance of Change Orders within the scope ofthe project and any such Change Orders shall be in writing.5.0EXISTING CONDITIONS5.1Quoters are cautioned not to submit their quotes until said plans, specifications, and profiles have been carefullyexamined by them.5.2Each quoter shall fully acquaint himself with conditions relating to the scope and restrictions attending theexecution of the work under the conditions of this quote.5.3It is expected that this will sometimes require on-site observation.5.4The failure or omission of a quoter to acquaint himself with existing conditions shall in no way relieve him of anyobligation with respect to this quote or to the contract.6.0BID BOND (Not Required)7.0PERFORMANCE/PAYMENT/MAINTENANCE BOND (Not Required)8.0COST OF PLANS (N/A)9.0AWARD CRITERIA9.1Award will be made to the lowest responsible and responsive quoter(s) according to the criteria designated in theInvitation for Quote.9.2In addition to price, the Quote Evaluation will include the following factors (as they apply):9.2.1The quality of performance/workmanship of previous contracts, or references which attest to the specificexperiences of others.9.2.2The timely completion of previous contracts, or references which attest to the specific experiences of others.9.2.3The sufficiency of financial resources and its impact on ability of the quoter to perform the contract.Page 4 of 32CITY OF SHREVEPORT SPECIAL INSTRUCTION/PROVISIONS CONTINUED9.2.4The City reserves the right to conduct on-site inspections of any quoter’s facilities prior to award and the results ofsaid inspection will be considered by the City in determining Quoters’ capabilities of successfully administering tothis contract.9.2.5The ability and availability of the quoter to provide quality and timely maintenance, service, and/or parts.9.2.6The cost of maintenance and operational costs.9.2.7The availability and capability of local support as it affects the quantity, quality, and timeliness of the work required.9.2.8The timely completion of a project as stated in the quote.9.2.9Substantial compliance or noncompliance with specifications set forth in the quote as determined by the City.9.2.10 Product or parts inventory capability as it relates to a particular quote.9.2.11 Results of product/equipment testing9.2.12 The warranty - Terms and Conditions.9.2.13Adequate capital and credit rating sufficiently to complete all operations under this contract in a satisfactorymanner.9.2.14An efficient office force, with a satisfactory record in expediting delivery of materials to field force and capable offulfilling proper liaison service with mechanical trades.9.2.15 An adequate and efficient field force, with extensive knowledge of all types of work involved under this contract.9.2.16 A record of amicable relations with labor.9.2.17 An adequate supply of construction equipment in good operating condition.10.0ALTERNATES10.1If alternates are listed on the Quote Schedule, it is the intent of the City, if the City accepts any Alternates, toaccept them in the order in which they are listed in the quote form.10.2The low quoter shall be determined based on the sum of the base quote and the Alternates accepted.11.0AWARD11.1The Purchasing Division will make award to one quoter for the total job.12.0REJECTION12.1This solicitation does not commit the City of Shreveport to award a contract, to pay any costs incurred in thepreparation of a quote, or to procure or contract for the articles of goods or service.12.2The City reserves the right to waive minor informalities, to accept or reject any or all quotes received as a result ofthis request, or to cancel in part or its entirety this quote, if it is in the best interest of the City to do so.12.3The City of Shreveport reserves the right to declare any quote non-responsive in which the delivery/completiontime indicated is considered to delay the operation for which the item/work is intended, due to the noncomplianceof the SPECIFICATIONS.13.0REJECTION OF LOWEST QUOTE13.1Substantial negative findings from the Award Criteria, as listed above, may result in the disqualification of thelowest quoter, if in the best interest of the City of Shreveport.14.0DISQUALIFICATION REVIEW BOARD (La. R.S. 38:2212 J- 1 & 2/ City of Shreveport Code of Ordinances Sec.26-265)14.1When a contractor has been given notice of possible disqualification based upon La. R.S. 38:2212 et. seq., and/ordebarment based upon Sec.26-265, the contractor may submit a written appeal to the Purchasing Agent forreview by the City’s Disqualification Review Board.14.2The written appeal must be submitted within ten (10) days after notice of possible disqualification and may requesteither (1) a meeting with the Review Board, or (2) that the Review Board consider a written appeal only.14.3A meeting of the Review Board will be scheduled within ten (10) days after receipt of the appeal.Page 5 of 32
