It shall be theconsultant’s/contractor’s sole responsibility to comply with all applicable economic sanctions.Cosumnes Community Services DistrictFD-27-001 Debt Collections ServicesSECTION 1 - PURPOSEThe Cosumnes Community Services District (“District”) is soliciting proposals from qualified vendors toprovide contracted debt collection services for ambulance transport, fire prevention, and relatedservices delivered by the Cosumnes Fire Department. The selected proposer must demonstrateestablished experience in delivering debt collection services in compliance with federal, state, and localregulations.SECTION 2 - DISTRICT PROFILEThe District is located in California’s Central Valley, within the southern part of Sacramento County,approximately four miles south of the Sacramento City limits. The District is the largest communityservices district in California, encompassing 157 square miles and serving an estimated population of210,000 residents.The District serves as a regional agency providing fire protection services to the cities of Elk Grove andGalt, as well as to unincorporated areas in southern Sacramento County. Additionally, the Districtprovides parks and recreation services exclusively to the Elk Grove community.The District is committed to improving the quality of life for residents, businesses, visitors, andemployees in the diverse community by protecting lives, property, and the environment through superiorfire suppression, emergency medical services, fire prevention, and special operations response; andby providing well-maintained parks and recreational opportunities that support health, wellness, andsocial interactions.The District is comprised of four main departments: Office of the General Manager Finance and Information Technology Department Parks & Recreation Department Fire DepartmentFire DepartmentThe Fire Department saves lives and protects the community through prevention, preparedness, andtimely, courteous, and effective emergency response. The submitting vendor(“Proposer”) must ensure that the product submitted meets or exceeds the minimum requirementsdetailed in Exhibit A - Scope of Service.Award of ContractThe District will award the contract to the Proposer, or Proposers, that is most advantageous to theDistrict. The Board of Directors of the Cosumnes Community Services District reserves the right toreject any or all Proposals and to waive any informality in any Proposal. Furthermore, the District reserves the right to cancel or reissue the RFP at its solediscretion. In the event of an amendment, the District will notify all Proposers of the changes and publishthe notice on its official website at www.CosumnesCSD.gov/Bids. Proposers are expected to respondto the final written RFP, along with any exhibits, attachments, and amendments thereto.RFP Questions - Directions for SubmissionAll specific inquiries regarding the Request for Proposal (RFP) should be directed via electronic mail tothe designated RFP Coordinator before the “Deadline for Questions.” Such inquiries must preciselyspecify the relevant section of the RFP and include the page number(s) relevant to the question. Boththe submitted questions and the District’s responses will be published on the District’s official websiteat www.CosumnesCSD.gov/Bids.Proposal SubmittalProposers must submit their proposals no later than the date and time specified in the RFP timeline.Submissions received after this deadline will not be considered. Incomplete submissions will also beineligible for consideration. All submissions must be transmitted electronically to the RFP Coordinator.Any submission made to the District via email will be acknowledged with a confirmation reply duringregular business hours. The RFP Coordinator will send an email confirming receipt of the submission.If a Proposer does not receive such confirmation, they must contact the RFP Coordinator prior to theproposal deadline.SECTION 5 - PROPOSAL SUBMISSION REQUIREMENTSThe following guidelines are provided for standardizing the preparation of RFP submissions. The intentis to assist Proposers in the preparation of their submissions and to assist the District in simplifying thereview process by providing standards for comparison of submissions.Submissions in response to this RFP must include a complete response to the requirements in thissection in the order presented. Submissions should be a straightforward delineation of the Proposer’scapability to satisfy the intent and requirements of this RFP and should not contain redundancies orconflicting statements.Proposal Submission Format and ContentProposals should be prepared on standard 8 ½" x 11" paper. All pages should be numbered. Failure tofollow this specified format, to label the responses correctly, or to address all subsections may, at theDistrict’s sole discretion, result in rejection of the submission.Proposals must contain the following information in the order listed:1. Proposer’s legal name and corporate structure.b. Proposer’s primary contact information, including name, address, phone, website, and email.c. Identification of use of subcontractors and scope of work to be performed by subcontractors.d. Identification of any pending litigation against the Proposer.e. Disclosure of any bankruptcy or insolvency proceedings in the last ten (10) years.f. Statement indicating the proposal will remain valid for at least 120 days.g. Signature of a company officer who is authorized to bind the Proposer to the provisions of thisRFP and any contract awarded pursuant to it.2. Executive SummaryThis section of the submission should provide a concise synopsis of the Proposer’s qualificationsand credentials to deliver the services sought under the RFP. In addition, this section should explainhow the proposed solution will differentiate itself from other vendor solutions and why the Districtshould select it. This may include a list of unique features that give the Proposer a competitive edge.This section should not exceed two (2) pages.3. Qualifications and ExperienceIn this section, the Proposer should identify the vendor/company and the personnel responsible forproviding services, including their qualifications and experience. List types and locations of similarwork performed by the Proposer in the last five (5) years that best characterize the quality and pastperformance.4. Work Plan and ServicesThis section should provide a detailed work plan for the services listed in Section 3 – Scope of Workof this RFP. This should include an overview of the Proposer's qualifications to perform eachcomponent as detailed in Exhibit A - Scope of Service.5. ReferencesInclude a minimum of three (3) references for comparable public agencies receiving similar servicesfrom the Proposer. References may be contacted during the review and selection process. For eachreference, the Proposer must provide the following:a. Name and contact information (i.e., name, title, agency, address, phone, and email)b. Brief description of the work provided6. Commitment to Diversity, Equity, and InclusionThe District is committed to ensuring that diversity, equity, and inclusion are integral parts of day-to-day management, work, and service delivery. The District’s Board approved the Equity statement,which is within District Policy “Commitment to Diversity, Equity, and Inclusion.” Proposers shouldreview the statement available at www.cosumnescsd.gov/DEI. Proposers should detail how theyensure their commitment to and detail their culture of diversity, equity, and inclusion, which guidestheir principles across their culture and operations. In this section, the Proposer must state andprovide a detailed narrative of any diversity, equity, and inclusion programs offered by theirorganization.4
