PURPOSEThe City of Unalaska (hereafter “City”) is seeking to retain a qualified professional safety consultant withqualifications and expertise to provide safety program consulting services to the City.The City requests the services of a safety consultant to help develop and implement all required safetyprograms to ensure the City remains in compliance with OSHA and AKOSH standards and protect thehealth and well-being of its employees. The consultant will work closely with risk management,department management and City administration to provide services as outlined in the scope of services.The City will enter into a Professional Services Agreement (see Attachment A, Form of Agreement andAttachment B, Insurance Requirements) with one individual or firm to perform an agreed-upon specificscope of services and fees that best meet the safety program needs of the City.II. BACKGROUNDUnalaska is the 12th largest incorporated city in Alaska and is 800 air miles southwest of Anchorage in theheart of the healthy and robust North Pacific/Bering Sea fisheries. Our community is a vibrant mix ofindustry and history connected by 44 miles of roads linking our port, harbors and private docks with localbusinesses and our thriving residential community of 4,120. We work hard to provide steady support toone of the busiest and most prosperous stretches of coastline in Alaska.Travel is primarily via air, so this geographical remoteness creates many unique challenges that requirecreative solutions. Three years ago, the arrival of high speed internet service to our island community wasa game changer, and we are building upon the opportunities that are now possible.The City has a Mayor-Strong City Manager form of government, which consists of a Mayor and six CityCouncil Members who are elected, and hire the City Manager. The city provides a wide range of services,which include:• City Hall: Finance, Information Services, City Manager’s Office, Planning, City Clerk, andAdministration (Human Resources and Risk Management)• Parks, Culture and Recreation: Public Library, Aquatics Center, Community Center, Parks andOutdoor Facilities• Police: Corrections, Communications, Animal Control and DMV• Fire and Emergency Medical Services• Public Works: Engineering and Administration, Supply, Roads, Vehicle Maintenance, FacilitiesMaintenance• Public Utilities: Electric Production, Electric Distribution, Water, Wastewater, Solid Waste• Ports and Harbors: Port and Harbor Facilities, Airport TerminalBecause the City of Unalaska is a small local government in a remote location, proposals should utilizecreative, cost-effective solutions. While price is a significant factor, a quality safety and training programare the goal and will form the basis of the contract award decision.1III. SCOPE OF SERVICESThe City is accepting proposals from qualified consultants to provide safety program consulting, trainingand assistance. The consultant shall provide qualified safety professional(s) who can provide on-siteservices periodically at the City, and remote support as needed. The scope of services shall include butare not limited to the items noted below.If the consultant feels that additional tasks are warranted, they should be clearly identified in theconsultant’s proposal.Task #1: Assess the City’s current safety programs1. Assess the City’s safety programs and identify deficiencies.Task #2: Assist city staff in correcting deficiencies and developing acomprehensive safety program that includes the following OSHA requiredelements1. Job Hazard Assessments: Assist with completion of written job hazard assessments todetermine/clarify/update the required safety training, personal protective equipment, shoes,gear, etc. for each city position.2. Written Plans: Written safety plans to ensure compliance with federal and state standards,regulations, laws and city ordinance and rules, as well as assist in the development of any newplans as needed. This includes, but is not limited to:a. Accident Investigationb. Aerosol Transmissible Diseasesc. Asbestos and Leadd. Bloodborne Pathogen and Exposure Control Plane. Chemical Hygiene Planf. Bilingual options (Tagalog, Samoan, Spanish, Vietnamese, etc.)Task #3: Develop a Facility Safety Inspection and Compliance Process1. Provide staff training to identify hazards (physical, equipment and behavioral).2. Establish inspection procedures, and best practices to evaluate facilities to assess risk andcompliance with federal, state, and local regulations.3. Develop hazard reporting forms and steps to correct the hazard.Task #4 Leadership Training for Supervisors, Managers and Safety Committee1. Safety Committee: Assist in developing safety leadership skills and how to create a safety culture.Provide guidance for safety program development and implementation. Recommend safety-related committee projects.2. Managers/Supervisors: Assist in developing leadership skills and how to create a safety culture.Provide guidance for safety program development and implementation. Recommend safetyincentives and how to encourage safe behavior in the work environmentTask #5 Provide Ongoing Support1. Annually participate in updating the master safety plan.2. While working with the city, provide a monthly progress report to Risk Management.3. Proposers are encouraged to recommend any additional options, recommended services, ortasks.3IV. PROPOSAL REQUIREMENTSProposal shall contain the following:1. Cover Letter. The Cover Letter shall be addressed to Debra Hanson Zueger, Risk Coordinator, andat minimum, must contain the following:a. Identification of firm, including name, address, and telephone number of contact personduring period of proposed evaluation;b. Statement to the effect that the proposal shall remain valid for a period of not less than120 calendar days from the date of submittal; andc. Signature of a person authorized to bind the firm to the terms of the proposal2. Executive Summary. In a brief narrative, summarize the proposed solution by setting forth theoverall approach and plans to meet the requirements of the tasks. The intent of this narrative isto convey to the City that the firm understands the objective of the requested service, the natureof the work, and the level of effort necessary to successfully provide the defined services.3. Scope of Work/Methodology. For tasks 1-4 above, provide a detailed description of the approachand methodology to be used to accomplish the tasks. The methodology should include:a. A detailed description of the approach to accomplishing the task.b. the specific tasks and roles required by City staff.4. Rates. Provide the consultant’s rate schedule and written statement that said rate schedule is partof the consultant’s quote for use in invoicing for progress payments and for extra work incurredthat is not of this RFP.5. Schedule. A tentative schedule of the amount of time for the project/tasks. The exact dates willbe determined jointly by the consultant and the city.6. Required Statementsa. A written statement by the consultant that all federal laws and regulations shall beadhered to notwithstanding any state or local laws and regulations. In case of conflictbetween federal, state, or local laws or regulations, the strictest shall be adhered to.b. A written statement that consultant will not discriminate against any employee orapplicant for employment because race, color, religion, sex, or national origin.c. That the consultant will provide all necessary personnel, and list what materials,equipment and staff shall be provided by the consultant and what materials, equipmentand staff shall be supplied by the City to perform the services described.7. Referencesa. Provide at least three (3) references using the same service being proposed and indicatethe scope of work, date, name, email address, and telephone number of the clientcontact.4
