1. Name, mailing address, email address, telephone number and fax number of the primary contact person for firm;
2. A brief description of firm, including the number of years in business, major business lines, major markets served, company history, relevant operating segments, primary vision and strategy, number of employees, office locations and any Joint Venture partners;
3. A copy of any resolution or some other form of authority, signed by a Chief Executive Officer, Corporate Secretary, or managing partners, which lists the specific officers who are authorized to execute agreements on behalf of the Offeror;
4. Financial details demonstrating your firm’s financial capacity to undertake and complete the services solicited herein;
5. Each Offeror must provide references from at least three (3) current customers or other governmental entities where Offeror has provided similar services for a minimum of three (3) years. At a minimum, the following information must be included for each client’s reference:
a. Client name, address, contact person name, telephone, and fax number;
b. Description of contract, provided that they are similar to the services outlined in this Request for Proposals;
c. Total dollar value of the contract;
d. Contract term (start and expiration).

GENERAL INFORMATIONAn RFP defines the situation for which the services are required, how they are expected to beused and/or problems that they are expected to address. Proposers are invited to submitsolutions which will result in the satisfaction of the County’s objectives in a cost-effectivemanner. The proposed solutions are evaluated against a predetermined set of criteria of whichprice may, but may not always be the primary consideration.Formal requests for clarification or questions regarding this Request for Proposal should besubmitted via email to the COUNTY at dfarberman@wyomingcountyny.gov by March 31, 2026by 4:00 PM. Any information given to a Proposer in response to a formal request will befurnished to all Proposers as an amendment to this solicitation if such information is deemednecessary for the preparation of uniform proposals. Only such amendments when issued by theCOUNTY will be considered as being binding on the County. Verbal explanations or instructionsgiven by a County employee, consultant, etc., to a Proposer regarding this solicitation shall notbe binding on the COUNTY, and shall be considered informal unless confirmed in writing by theDirector of Human Resources.While it is the County’s express intention to issue a fair and competitive document, the Countyreserves the right to waive specific requirements of this solicitation when such waiver isdeemed to be in the County’s best interest. This Request for Proposal does not commit Countyto award a contract or pay any costs incurred by a Proposer in the preparation of a proposal.Any Proposer who submits a proposal in response to this solicitation does so at their ownexpense including attendance at subsequent meetings to discuss their proposal.1. Conditions, Specifications, and Stipulations of Proposals.The purpose of the Proposal is to demonstrate the qualifications, service level, cost for services,competence, and capacity of the firms seeking to become our EAP provider. The proposalshould address all the points outlined below.The County reserves the right to make whatever investigations or inquiries necessary todetermine the competency and ability of any Proposer to complete the requirements of thissolicitation. A review may include but not be limited to inspection of the Proposer’s facilitiesand equipment, references or previous contract performance with the County or others.2. Evaluation - Award.All proposals received in response to this RFP will be evaluated to determine if they arecomplete and meet the requirements specified in this RFP. After determining that a proposalsatisfies the requirements stated in the RFP, a comparative assessment of the relative benefitsand deficiencies of the proposal in relationship to the published evaluation criteria will be madeusing subjective judgement. The award of a contract shall be based on the best proposalreceived in accordance with the evaluation criteria described below, and whose offer is judgedto be the most advantageous to the County. If needed, additional information, and/orinterviews may be requested.3. Project - DeliveryForm of ContractAll proposals received in response to this RFP will be evaluated to determine if they arecomplete and meet the requirements specified in this RFP. After determining that a proposalsatisfies the requirements stated in the RFP, a comparative assessment of the relative benefitsand deficiencies of the proposal in relationship to the published evaluation criteria will be madeusing subjective judgement. The award of a contract shall be based on the best proposalreceived in accordance with the evaluation criteria described below, and whose offer is judgedto be the most advantageous to the County. If needed, additional information, and/orinterviews may be requested.The successful Offeror(s) if any, will be awarded a three (3) year contract. Subsequently, theagreement may be renewed, upon mutual agreement of the parties, for three (3) additionalperiods of one (1) year each. Any extension of the agreement must be in writing and the onlycondition that shall be permitted to be altered or changed in any extension is the sum to bepaid the offeror and the method of computing the same. The county reserves the right tocontract any services awarded under this RFP to other firms at its sole discretion whenever itdeems necessary. The contract with the successful Offeror (if any) shall include the terms of thisRFP together with those terms of the Offeror’s proposal, which are not inconsistent with theRFP, and which have been specifically accepted by the County. Consideration for renewing thecontract will be determined by the Department of Human Resources following its analysis ofthe Offeror’s performance.Contractor’s CompensationAll proposals must contain relevant information for the factors listed above along with theproposed fee, including but not limited to all expenses, hourly staff rates, payment terms, andcontract terms required by Offerors and conditions and options. The Pricing Sheet containedherein must be completed and returned with the proposal. Offeror must specify the overall costfor providing all of the services described herein.5. Scope of Services.The proposal should include the following requirements in order to be accepted.If a proposal identifies an additional element not included in these specifications, which in itsjudgement would be an essential element to accomplish the intended objectives as articulatedin this proposal request, the proposer should identify this element in its proposal and explain indetail why the County should consider including this element within the Scope of Services.Conversely, if a proposal identifies a task within the Scope of Services, which it believes could bemodified or deleted without impacting the objectives of the proposal, the proposer shouldprovide an explanation as to why the task should be deleted or modified. The County reservesthe right to accept or reject the addition, deletion or modification of an element of thesespecifications. Counseling Benefits including but not limited to: Face to Face Counseling Sessions, Telephonic Counseling, Digital Therapies,Digital Mobile App, Drug, Alcohol and Substance Abuse Counseling Work-Life Benefits included but not limited to: Legal, Financial, Caregiver Benefits, Adoption Benefits and Personal ResearchAssistant. Learning and Development included but not limited to: Personal and Professional Online Training, Personal Education Center andMonthly Webinars. Administration Services included but not limited to: EAP Member and Orientation, Ongoing Communication and Mobile Website. Other Services included but not limited to: Human Resource Consultations, Administrative Referral, Drug Free Workplace,Trauma Response Services including in person and phone consultations.6. General Conditions:Wyoming County will review and evaluate the proposals from qualified applicants.
