Expensive bindings, color displays, and the like are notnecessary or desired. Emphasis should be placed on conformity to the RFP’s instructions andrequirements, and completeness and clarity of content.7.2 The Proposer must submit its proposal in two parts, the non-cost portion and the cost portion.a. The Proposer must submit one (1) copy of the non-cost portion of the proposal. The originalmust be signed by an authorized representative of the Proposer. The original non-cost portionof the proposal (and the copies thereof) must be submitted to the Court in PDF or word format.b. The Proposer must submit one (1) copy of the cost portion of the proposal. The original mustbe signed by an authorized representative of the Proposer. The original cost portion of theproposal (and the copies thereof) must be submitted to the Cour in PDF or word format.6.3 Proposals must be delivered by the date and time listed on the coversheet of this RFP to:solicitationsmailbox@scscourt.org6.4 Late proposals will not be accepted.7.0 PROPOSAL CONTENTS7.1 Non-Cost Portion: The following information must be included in the non-cost portion of theproposal. A proposal lacking any of the following information may be deemed non-responsive.a. The Proposer’s name, address, telephone and fax numbers, and federal tax identificationnumber. Note that if the Proposer is a sole proprietor using his or her social security number,the social security number will be required before finalizing a contract.b. Name, title, address, telephone number, and email address of the individual who will act as theProposer’s designated representative for purposes of this RFP.c. Model number(s), specifications, or other description of the goods the Proposer proposes tosupply to the Court, including warranty information.d. Names, addresses, and telephone numbers of a minimum of three (3) clients for whom theProposer has provided similar services. The Court may check references listed by theProposer.e. For each key staff member: a resume describing the individual’s background and experience,as well as the individual’s ability and experience in conducting the proposed activities.f. Acceptance of the Terms and Conditions.(1) On Attachment 3, the Proposer must check the appropriate box and sign the form. If theProposer marks the second box, it must provide the required additional materials. An“exception” includes any addition, deletion, or other modification.(2) If exceptions are identified, the Proposer must also submit (a) a red-lined version of theTerms and Conditions that implements all proposed changes, and (b) a writtenexplanation or rationale for each exception and/or proposed change.(3) Note: A material exception to a Minimum Term will render a proposal non-responsive.g. Certifications, Attachments, and other requirements.(1) The Proposer must complete the General Certifications Form (Attachment 4) and submitthe completed form with its proposal.GS-036 IT REV 10/18/24Page 4 of 27RFP Title: Microsoft Azure SubscriptionRFP Number: 103125C(2) The Proposer must submit with its proposal, for itself and each of its affiliates that makesales for delivery into California, a copy of either (i) a California seller's permit issued underRevenue and Taxation Code section 6066 et seq. or (ii) a certificate of registration issuedunder Revenue and Taxation Code section 6226.(3) If Contractor is a California corporation, limited liability company (“LLC”), limitedpartnership (“LP”), or limited liability partnership (“LLP”), proof that Contractor is in goodstanding in California. If Contractor is a foreign corporation, LLC, LP, or LLP, andContractor conducts or will conduct (if awarded the contract) intrastate business inCalifornia, proof that Contractor is qualified to do business and in good standing inCalifornia. If Contractor is a foreign corporation, LLC, LP, or LLP, and Contractor doesnot (and will not if awarded the contract) conduct intrastate business in California, proofthat Contractor is in good standing in its home jurisdiction.(4) The Proposer must complete the Unruh Civil Rights Act and California Fair Employmentand Housing Act Certification (Attachment 8) and submit the completed certification withits bid.7.2 IT Services:a. A detailed line item budget showing total cost of the proposed services.b. A full explanation of all budget line items in a narrative entitled “Budget Justification.”c. A “not to exceed” total for all work and expenses payable under the contract, if awarded.NOTE: It is unlawful for any person engaged in business within this state to sell or use any articleor product as a “loss leader” as defined in Section 17030 of the Business and Professions Code.8.0 OFFER PERIODA Proposer's proposal is an irrevocable offer for ninety (90) days following the proposal due date. In theevent a final contract has not been awarded within this period, the Court reserves the right to negotiateextensions to this period.CRITERIONNOTE: THESE ARE SAMPLE CRITERIA. The interview process may require a demonstration. Theinterview may also require a demonstration of equivalence if a brand name is included in the specifications.The interviews may be conducted in person or by phone. If conducted in person, interviews will likely beheld at the Court’s offices. The Court will not reimburse Proposers for any costs incurred in traveling to orfrom the interview location.
