Proposalssubmitted after this time shall not be accepted. Any claim by a Contractor of error in its proposal must be made incompliance with section 5100 et seq. of the Public Contract Code.The prequalification process shall be done through PQBids. The prequalification process must be completed prior tosubmitting a proposal. The prequalification process can be completed at: https://pqbids.com/carlsbadDIR Registration: Contractors submitting proposals are required to register as a Public Works Contractor with theCalifornia Department of Industrial Relations. Registration must remain active throughout the term of the contract.CUPCCAA Registration: pursuant to Public Contract Code Section 20111.5, only pre-registered Contractors will be eligibleto submit a bid for this project. Any bid submitted by a Contractor who is not pre-registered will not be accepted.Contractors can register on the District’s website at: https://pqbids.com/carlsbadProject Description: Carlsbad USD intends to purchase and install new network equipment and device licenses at sevenschool sites to refresh existing devices. The project includes vendor installation services, removal of current devices, andprovision of materials and license details, with installation coordinated by the District’s Technology and Assessmentteam.Minimum Qualifications: except as provided herein, a proposal submitted to the District by a Contractor who is notlicensed as a Contractor pursuant to the business and professions code shall be considered non-responsive and shall berejected by the District. Where federal funds are involved the Contractor shall be properly licensed at the time the contractis awarded. In all other cases the Contractor shall state their license number, expiration date and classification in theproposal, under penalty of perjury. The following classifications are acceptable for this contract:C-7 or C-10Must be an authorized vendor registered with the USAC (Universal Service Administrative Company) at time of servicesrendered. The project herein is contingent upon the approval of funding from the Universal Service Fund’s Schools andLibraries Program, otherwise known as E-rate. Questions must bereceived by Friday, December 5, 2025, 4PM PST. The references should be local to the San Diego region. Provide the customer’sbusiness name, name of contact person, title, phone number and email address of the individual at each reference sitewhom we can contact for information. Inform your references that we may be contacting them to discuss yourperformance if you are among those selected for consideration.PREPARATION OF PROPOSAL FORMPlease prepare your proposals on the form attached to be submitted at such time and place as is stated in the Notice toVendors calling for proposal. All blanks in the Proposal Form must be appropriately completed. All proposals must besubmitted and received by the District through PlanetBids no later than Friday, December 19, 2025 at 2:00pm. Proposalssubmitted after this time shall not be accepted. Any claim by a Contractor of error in its proposal must be made incompliance with section 5100 et seq. of the Public Contract Code.Respondents shall be solely responsible for examining the RFP and the Contract Documents, including any addendaissued during the bidding period, and for informing itself with respect to local labor availability, means of transportation,necessity for security, laws and codes, local permit requirements, wage scales, local tax structure, licensingrequirements, availability of required insurance, and other factors that could affect the work. Respondents areresponsible for consulting the standards referenced in the included sample Contract. Failure of respondent to so examineand inform itself shall be at its sole risk, and no relief for error or omission will be given except as required under Statelaw.09-2526A E-Rate Network Switch Upgrade4Encyclopedic proposals which do not specify the products or services that will meet the scope and requirementsspecified may be disqualified.BRAND NAMES AND QUANTITY1. Brand names and quantity on the Specifications and Proposal Form are for reference. If a Service Providerchooses to bid another manufacturer than is preferred, please provide supporting documentation with your bidresponse demonstrating its functional equivalency. Evaluation of equivalency is at the sole discretion of the District.Proposals on equivalent items will be considered, provided the proposal clearly describes the article offered and it isequivalent in quality, warranty, lifecycle costs, and utility.2. State brand and model on each item. If proposing other than the make, model, or brand specified in thespecifications, state the item offered by the manufacturer’s name and model number. Unless the proposer clearlyindicated in his proposal that he is offering an “equal” product, his proposal shall be considered as offering the brandname product referenced in the Specifications and Proposal Form.PROPOSE SEPARATELYPropose on each item separately. Prices should be stated in units specified on the Proposal Form. If standardpackaging is not consistent with the proposal, so indicate on the Proposal Form.SIGNING OF PROPOSALSThe signature of all persons signing shall be in longhand or in an electronic format that complies with applicablestate and federal electronic signature laws and executed by principal duly authorized to make contracts. Theproposer’s legal name shall be fully stated. Obligations assumed by such signature must be fulfilled.QUALIFICATIONSThe District expects Service Providers to make themselves thoroughly familiar with any rules or regulations regarding theE-rate program. Service Providers are required to be in full compliance with all current requirements and futurerequirements issued by the SLD throughout the contractual period of any contract entered into as a result of this RFP.Service Providers are responsible for providing a valid SPIN (Service Provider Identification Number). More informationabout obtaining a SPIN may be found at this website:https://www.usac.org/e-rate/service-providers/step-1-obtain-a-spin/Service Providers are responsible for providing a valid Federal Communications Commission (FCC) RegistrationNumber (FRN) at the time proposal is submitted. More information about obtaining a FRN may be found athttps://fjallfoss.fcc.gov/coresWeb/publicHome.doService Providers are responsible for providing evidence of FCC Green Light Status at the time the proposal issubmitted. Any potential bidder in Red Light Status must provide an explanation of the steps it is undertaking tobe removed from Red Light Status and the timeframe for resolution. More information about FCC Red and GreenLight Status may be found at http://www.fcc.gov/debt_collection/welcome.htmlAll proposers may be required to furnish evidence of their technical ability, experience, and financialresponsibility.
