The City of Surprise (City) Fire-Medical Department (SFMD) is seeking Statements of Qualifications (SOQ) under this Request for Qualifications (RFQu) for a qualified Medical Director (MD), either independently or in cooperation with an agency, to provide essential clinical oversight and guidance for the Emergency Medical Services (EMS) operations. The MD, or their qualified designee, will be responsible for reviewing, approving, and maintaining medical protocols that reflect current best practices and ensuring the competency of all EMS personnel through continuous training and credentialing. The role includes providing real-time medical oversight, when necessary, to support critical decision-making in the field.

The MD, or their qualified designee, will also ensure compliance with all state and federal regulations, maintain strong relationships with receiving hospitals, and foster seamless coordination for patient transfers. SFMD is committed to excellence in patient care, and the MD, or their qualified designee, will play a pivotal role in achieving this goal.

It is preferred that the MD, or their qualified designee, also participate in the Continuing Education (CE), Controlled Substances, and Continuous Quality Improvement (CQI) programs. This includes reviewing patient care data, identifying areas for improvement, and assisting in developing strategies to enhance the quality of care delivered by the EMS personnel.

Background

Established in 1960, the SFMD serves a rapidly growing and diverse community in the West Valley of the Phoenix metropolitan area. Originally a volunteer organization, the SFMD has evolved into a robust department with eight fire stations strategically located throughout the city, covering over 130 square miles and a population exceeding 165,000 residents.

The department has 230 full-time employees and a budget of $48.8 million for fiscal year 2025, with specific allocations for different services and operations. T his dynamic environment presents unique challenges and opportunities for the delivery of pre-hospital medical care. The City operates under a council-manager form of government, with oversight of the SFMD provided by Fire Administration. The MD, or their qualified designee, will play a crucial role in overseeing EMS clinical protocols, training, and quality improvement to ensure high standards of patient care. They will assist in developing and updating medical guidelines, reviewing patient outcomes, and providing feedback for continuous improvement. The MD, or their qualified designee, also ensures EMS members are properly credentialed and that the department complies with healthcare regulations. Additionally, they will collaborate with local hospitals and public health agencies to improve community health and support EMS personnel.

Fire-Medical Department Overview

The Surprise Fire Medical Department (SFMD) has a variety of responsibilities, including fire prevention, fire investigation, fire suppression, emergency medical services, hazardous material response, technical rescue, and emergency preparedness activities. The SFMD operates from eight fire stations and supports eight engines, one ladder, six ambulances, and two battalion chiefs, all with advanced life support capabilities.

The Phoenix Fire Department dispatches all department resources as part of a valley-wide automatic aid partnership, ensuring that the closest available unit is dispatched to incidents regardless of municipal location. This system provides fire suppression, emergency medical services, and other emergency rescue services to 5 million people. In 2024, the SFMD responded to 22,469 incidents system-wide, with 19,535 of these calls located within the City of Surprise.

In 2019, the SFMD was evaluated by the International Organization for Standards (ISO) office and was rated a Class 1 in the urban areas and a Class 3 in the city's rural area. The Department has diligently worked over the last decade to reduce its ISO rating and aspires to achieve a Class 1 status citywide. Despite some rural locations within the City of Surprise experiencing longer response times due to current roadway configurations, sparse population, and location of fire stations, the SFMD is committed to meeting the National Fire Protection Association's (NFPA) 1710 guidelines where possible.
