Both projects will be part of one bid package. The estimated constructionduration is 275 calendar days.Prequalification applications will be available on or about April 14, 2026, at the district website. All subcontractors that intend to bid to perform under the following license classification(s)as a first-tier subcontractor to a general contractor are required to be prequalified by the District: C-4, C-7, C-10, C-16, C-20, C-34, C-36, C-38, C-42, C-43, and/or C-46. Subcontractors must complete this District form; no other prequalification documentssubmitted by a Subcontractor will meet the District's requirements. Subcontractors must provide a minimum of three (3) DSA-regulated project references as detailed inPart 4.Project Size. For this Project, the District anticipates a construction value in excess of $20,000,000 and intends toprequalify only those Subcontractors whose prior experience demonstrates capacity to perform work ofcomparable complexity under DSA oversight. Subcontractors must demonstrate completion of a minimum of three(3) DSA-regulated projects with individual subcontract values of $500,000 or greater within the past five (5) years.BASSETT UNIFIED SCHOOL DISTRICTPREQUALIFICATION QUESTIONNAIREDOCUMENT 00 32 00Updates. Subcontractors who are prequalified must update their prequalification questionnaire if or when theSubcontractor's status or information changes from that previously disclosed to the District. The District reservesthe right to adjust, suspend, or rescind the prequalification rating of any Subcontractor based on subsequentlylearned information.Rejection/Waiver/Request. The District reserves the right, in its sole discretion, to reject any or all prequalificationquestionnaires, to waive irregularities in any prequalification questionnaire or to request further information ordocumentation from any Subcontractor.Public Records. Although the names of Subcontractors seeking prequalification may be public information,pursuant to, without limitation, Public Contract Code sections 20111.5(a) and 20111.6(b), each Subcontractor'squestionnaire and financial statements "shall not be public records and shall not be open to public inspection."However, the contents of Subcontractor's prequalification questionnaires and financial statements may bedisclosed to third parties for purposes of clarification or investigation of material allegations or in any appealprocess.Prequalification of Bidders. The District requires from prospective subcontractors answers to questions containedin this Application including a complete statement of the prospective subcontractor's financial ability andexperience in performing public works, Division of State Architect (DSA) projects or projects with similar scope ofwork. These documents will be the basis of rating subcontractors to determine whether a subcontractor isqualified to bid on the Project.DIR Registered Subcontractor Status. Each Subcontractor must be a DIR Registered Contractor to qualify as a Pre-qualified Bidder. The Pre-Qualification Application of a Subcontractor who is not a DIR Registered Contractor willbe rejected as non-qualified.Community Workforce Agreement (CWA) Compliance. This District is subject to a Community WorkforceAgreement (CWA) that governs the terms and conditions of employment for certain construction work performedon district projects. All subcontractors of any tier awarded work under this prequalification must comply with therequirements of the CWA. The successful subcontractor will be required to execute a Letter of Assent prior tocommencing work, agreeing to be bound by the terms and conditions of the CWA. A copy of the CWA will be madeavailable for review as part of the bid documents.Contents of Application for Submission. The following Sections/Documents should be completed and submittedto be considered a completed Prequalification Application: Part 1: Subcontractor or Firm Information Part 2: Pass/Fail Questions Part 3: Scored Questions Part 4: DSA Project References (minimum 3 projects) DIR Registration Verification (Attachment 1)Questions Regarding Prequalification Application. All questions regarding the Prequalification Application orPrequalification process must be submitted no later than 1:00 P.M. on April 23, 2026. Questions will only beaccepted in writing and sent via email to Brianda Ramirez, brianda.ramirez@bassettusd.org and the District'sSenior Director of Project Management, Robert Segarra, robert.segarra@cumming-group.com.Non-Responsiveness. Each Prequalification Application shall include those items listed above, any PrequalificationApplication not containing the above-referenced requisite documents completed with all information required andbearing the signature of the Subcontractor's duly authorized representative under penalty of perjury may renderthe Prequalification Application non-responsive.BASSETT UNIFIED SCHOOL DISTRICTPREQUALIFICATION QUESTIONNAIREDOCUMENT 00 32 00Waiver of Irregularities. The District reserves the right to waive minor irregularities and omissions in theinformation contained in the Prequalification Application submitted, to make all final determinations.Late Applications. The District may refuse to grant prequalification where the requested information is notprovided by the specified date and time. There is no appeal from a failure to submit an incomplete application orlate application.Confidentiality. The completed Prequalification Applications submitted by Subcontractors are not public recordsand are not open to public inspection. All information provided will be kept confidential to the extent permitted bylaw. However, the contents may be disclosed to third parties for purpose of verification, or investigation ofsubstantial allegations, or in the appeal process. State law requires, however, that the names of Subcontractorsapplying for prequalification status shall be subject to disclosure.Notification of Prequalification Results. Subcontractors will be notified by e-mail of their prequalification rating.Prequalification status will remain valid for one year from the date of District notification, except that the Districtreserves the right during the calendar year to adjust, increase, limit, suspend or rescind the prequalification ratingsbased on subsequently learned information and after giving notice of the proposed action to the prequalifiedSubcontractor.Prequalification Determination of Responsibility. While it is the intent of this Prequalification Application to assistthe District in determining bidder responsibility prior to bid, neither the fact of prequalification, nor anyprequalification rating, will preclude the District from a post-bid consideration and determination of whether asubcontractor has the quality, fitness, capacity and experience to satisfactorily perform the proposed work, andhas demonstrated the requisite trustworthiness.Prequalification Scoring/Evaluation Sections. To prequalify, each prospective subcontractor must meet or exceedthe requirements outlined in the scored sections below: Part 1 - Subcontractor or Firm Information. Fill out the information sheet fully. Part 2 - Essential Requirements for Qualification (Pass/Fail). The questions within this section are evaluatedutilizing pass or fail approach. Subcontractors that do not meet all requirements in this section will not bequalified to bid. Subcontractors receiving a fail score in this section will not be evaluated any further. Part 3 - Scored Questions. Evaluation Criteria: The Maximum score for this section is 115 points. Note thatmeeting the minimum score on the scored sections does not guarantee prequalification as there are non-scored questions that will also be objectively evaluated. Part 4: DSA Project References. Subcontractors must provide at least three (3) completed DSA-regulatedprojects with individual subcontract values of $500,000 or greater, completed within the past five (5) years,performed under the same license classification being prequalified. Failure to provide the minimum three (3)qualifying references results in automatic disqualification. The District reserves the right to contact referencesto verify information.The maximum total score for the Scored Questions section is 115 points. To be prequalified, a responsivesubcontractor must achieve a minimum combined score of 80 points. Meeting the minimum score does notguarantee prequalification.Note that meeting the minimum score on the scored sections does not guarantee prequalification as there arenon-scored questions that will also be objectively evaluated.Appeal. A Subcontractor may appeal the District's decision.
