RFP-2027-DCYF-02-ROADM: Roadmap to Reunification

Division of Children, Youth and Families

The New Hampshire Department of Health and Human Services, Division of Children, Youth and Families (“Department”) is seeking responses to this Request for Proposals ("Solicitation" or "RFP") from qualified Vendors to provide facilitation services for the Roadmap to Reunification Process to effectively engage reunification teams with planning for safety, permanency, and well-being of children and youth who reside in out-of-home care settings for whom the Department is legally responsible.

The Department anticipates awarding one (1) contract for the services in this Solicitation.
