This Addendum forms a part of the Contract Documents and modifies the original Bidding Documents.
Acknowledge receipt of this Addendum in the space provided on the Bid Form. Failure to do so may subject
the Bidder to disqualification. The following modifications are being made as described herein:
I. DAVIS-BACON WAGE RATES
A. Davis-Bacon wage rates have changed and the current modification must be included in the bid
documents. The updated wage rates (General Decision Number: OH20260001 01/02/2026) are
enclosed.
Please acknowledge receipt of this addendum by inserting these revised pages into the contract documents
and by indicating the Addendum No. and date on the Bid.
