Project DescriptionHeartland Behavioral Healthcare located at 3000 Erie St S, Massillon, Ohio is operated by the Ohio Department ofBehavioral Health. The facility operates 24/7, 365 days per year. This project will look to replace damaged sections of thefacilities’ sanitary sewer system. An assessment of the system was completed in 2023 and this project will look toimplement those recommendations. This project will also replace the failing waterline serving the facility. The line hasneeded constant repair over the last several years and will need to be replaced in entirety. A second source for feedingthe hospital will need to be explored and if possible, built into the loop.B. Scope of ServicesFor projects advertised with an appropriately developed Program of Requirements (“POR”), upon award of theAgreement, commence with Design. For projects without such a POR, upon award of the Agreement, commence bydeveloping the Program of Requirements.The selected A/E, as a portion of its required Scope of Services and prior to submitting its proposals, will discuss andclarify with the Owner and/or the Contracting Authority, the cost breakdown of the A/E Agreement detailed costcomponents to address the Owner’s project requirements. Participate in the Encouraging Diversity, Growth & Equity(EDGE) Program as required by statute and the Agreement.As required by the Agreement, and as properly authorized, provide the following categories of services: ProgramVerification, Schematic Design, Design Development, Construction Document Preparation, Bid and Award Support,Conformed Documents, Construction Administration, Post-Construction, and Additional Services of all types.Refer to the Ohio Facilities Construction Manual for additional information about the type and extent of services requiredfor each.
