Colored vinyl jacket with clear vinyl front pocket

Colored Jacket Width – 6 ¼ inches x Height 5 ¼ inches

Clear vinyl pocket Width 6 ¼ inches x Height 4 inches

On back of colored vinyl jacket is a magnetic strip with sufficient strength to hold File of Life and

multiple sheets of paper in the pocket securely to a magnetic surface.

Colors – vinyl jacket color is Purple

On top 1 ¼ inch of vinyl jacket on front side there will be an imprint that reads “File of Life” with a heart

beat logo.

On the clear vinyl pocket there will be 3 small imprints with logos at the bottom evenly spaced for

Cleveland EMS, Department of Aging, and Age Friendly Cleveland

Artwork to be provided.

Effective Immediately: In accordance with Codified Ordinance 181.13 (l) the City shall

receive competitive sealed bids through email. All City of Cleveland bids estimated to be

less than 50K in value will be submitted and received electronically via email. All bids

must be emailed to CityofClevelandbids@clevelandohio.gov. Upon receipt of your bid,

you will receive a reply indicating your bid has been received.
