Providers are to submit a Statement of Qualifications (SOQ) in accordance with theterms and conditions indicated below.2.0 BACKGROUNDThe Children’s Services Act is now the name of the Comprehensive Services Act for At-Risk Youth and Families (CSA), passed in 1992 and implemented in 1993, to provide foran innovative Systems of Care approach for Virginia’s children and families. The City ofSuffolk Family Assessment and Planning Team (FAPT) is responsible for identifying thevendors to provide services for individual cases.In addition, the Child Abuse Prevention and Treatment Act (42 USC Ch. 67) (CAPTA)enacted in 1974, reauthorized in 2010 and amended in 2015, 2016, and 2018 providesfederal funding and guidance related to child abuse neglect and passed in andimplemented. The Families First Prevention Services Act (FFPSA), enacted in 2018,provides additional guidance and funding related to preventing child abuse and neglectas well as unnecessary placement into foster care. Suffolk Social Services is responsiblefor identifying and connecting children and families with these services.3.0 TERM OF CONTRACTThe contract shall cover the period beginning July 1, 2026 (or upon receipt of signedContract) and continuing through June 30, 2028. This is an “as-needed” agreement. It isunderstood and agreed that the City shall not guarantee any minimum amount of work. Anycontract that may result from this solicitation shall not obligate the City to utilize the providerfor the services described.4.0 SCOPE OF SERVICESThe City is looking for vendors who can provide the following services:• Residential (congregate care) programs, Medicaid Level B & C• Private day schools and residential schools for students with disabilities• Shelter care• Treatment Foster Care• Independent living services• Transportation• Community based services (may include any of the following and other servicesas received in a community setting: Evidence-Based Programs for children andfamilies, Applied Behavior Analysis, Assessment/Evaluation, family supportservices, individualized support services, mentoring, respite, mental healthservices, substance abuse services, sex offender treatment services)• Other additional services as required4All providers who are funded using Children’s Services Act funding are required to attendFAPT meetings, provide a written treatment plans that correlate with the IndividualizedFamily Service Plan (IFSP) within 30 days of initiation of services, and provide monthlyprogress summaries. When providers are funded utilizing other funding streams,provider representatives are required to participate in Treatment Team, Child and FamilyTeam Meetings and/or Family Partnership Meetings as they apply to the families andchildren being served. Providers must be cooperative with utilization review proceduresconducted by the CSA staff and/or Social Services staff, and FAPT including allowingchart and on-site reviews and visitation with the child and staff providing services.Providers must actively participate with Suffolk Social Services and CSA staff related tochildren and families.For services whose pay rates are approved at the state or federal level, providers mustagree to accept the standardized rate as established by the Office for Children’sServices, the Virginia Department of Social Services or Medicaid.Vendors are also highly encouraged to participate in the annual Southeastern RegionCSA Vendor Fair in order to meet agency staff and discuss their services. Vendors maybring marketing materials during presentations but are not allowed to distribute items thatcould be construed as gifts.5.0 QUALIFICATIONSProviders must demonstrate that they have the resources and capability to provide theservices as described herein. All providers must submit documentation as indicatedbelow with their qualifications. Providers must be fully licensed and insured in theCommonwealth of Virginia. All providers who work directly with children and familiesare expected to ensure that employees have been background screened to include, at aminimum, a clear Child Protective Services check and criminal background check inaccordance with applicable law.A. Whether the provider is enrolled to provide services for Medicaid reimbursementB. Current licensure/certification with the appropriate organizationsC. Ability, capacity and skill of the provider to provide the services required,including verifiable competencies and accreditationD. Rates for services to be deliveredE. Providers are preferred to possess a clinical license for at least one service orhave supervision provided by a licensed person on staff.F. Insurance as defined by the RFQ per City of Suffolk requirementsG. Reference letters (at least two from other professionals)H. Performance data for prior fiscal year. If outcome data is not available, submitplan for collection of such data for the next fiscal year.5
