Category: Professional Services Status: Open Description: The Town of Apex is requesting Statements of Qualifications (SOQ) for professional and engineering services for the design and construction of a permanent stage with a roof, pivoting wood slat wall, conditioned storage, unconditioned storage, and ramps with lighting and sound options. The intent of this Request for Qualifications (RFQ) is to solicit a SOQ from architectural firms qualified and interested in the project. The Town of Apex plans to select a firm to design and produce construction documents, provide estimates, and perform construction administration services for the project using the Design-Bid-Build delivery method.

Responses received after the deadline above will not be accepted orconsidered.• None of the items submitted will be returned.• Responses submitted must be concise and address the objectives requested.• Responses shall be limited to a maximum of 15 single-sided pages, including items 1 through 10below• Statement of Qualifications shall address the following, at a minimum, and should conform tothe following numbering system:1. Letter of transmittal identifying contact person2. Statement substantiating firm’s understanding of project objectives and task assignments3. Company background and historyA. A brief history of the firmB. Resumes of the principalsC. Firm’s NC license number4. List similar design work performed by the company over the last 5 yearsA. Include, at a minimum: size, scope, cost, location, dates, time adherence, and ownercontact5. Identify the proposed project architect (person directly responsible for the design andproduction of working documents) and the proposed principal-in-charge and/or projectmanager (if different from the project architect)A. Include five projects the proposed project architect completed in the last five (5) years.For each project, include size, scope, cost, location, owner contacts, date completedversus original scheduled completion date, final actual cost versus initial budget, andpercent of change orders issued for the project. Projects listed in Number 4 above maybe included.B. Include resumes of project architect and principal-in-charge and/or project manager6. List the firm/consultant and individual(s) who will perform or be responsible for the closesupervision of the following design work:A. Architectural DesignB. Civil Engineering (including landscaping)C. Structural EngineeringD. Plumbing, Mechanical, Electrical Engineering and Fire Protection7. Provide three (3) client references (other than items 4 and 5 above). Local Governmentreferences are preferred. Provide one reference from a financial institution. Provide firm’sfinancial statement.8. Describe previous litigation or arbitration in which your firm has been involved and theoutcome during the past five (5) years.9. Describe your firm’s approach to construction administration. How will you ensure that thecontractor meets the owner’s objectives in terms of schedule, cost control, and qualitycontrol? List the person who will be providing construction administration and include theirresume.10. State any history or present condition that would put the firm in a conflict of interest in thedesign and/or construction of the project. Attach any supporting material to your response.Selection and AwardThe Town of Apex anticipates interviewing one or more firms before making a final selection.The Town of Apex shall select the most qualified firm and reserves the right to reject any and allresponses based on the following criteria:1. Demonstrated understanding of the project2. Demonstrated experience with public facilities3. Accessibility to the project site4. Ability of the firm to meet the project schedule and remain within the budget5. Such other appropriate information as the Town deems advisableThe Town of Apex will award the contract to the selected firm based on successful negotiation of the feefor this work.
