Services shall be executed strictlyin accordance with the specifications and conditions outlined herein.1.1 Schedule of Events1. All proposals shall be received in hard copy (printed) form no later than the dateand time shown in the Invitation to Proposers. Failure to meet the proposal’s opening dateand time shall result in rejection of the proposal.PROPOSALS SHALL BE OPENED PUBLICLY AT THE PHYSICAL LOCATION IDENTIFIED ABOVE AND ONLY THE NAMEOF THE PROPOSERS SUBMITTING PROPOSALS SHALL BE IDENTIFIED ALOUD. NO OTHER INFORMATIONCONTAINED IN THE PROPOSAL SHALL BE RELEASED OR DISCLOSED.2.2 Proposal Response ContentTo standardize and simplify the evaluation of responses, proposals should contain all of the following informationand be organized in the sequence indicated below. All of the sections should be appropriately labeled and boundtogether under a single cover not to exceed two (2) volumes with any identified appendices included as a separatevolume.A. Title Page: Title Page should show the Request for Proposal’s subject, proposal title, the firm’s name,address and telephone number of a contact person(s) who will address technical and contractual mattersthroughout the evaluation period; and the date of the proposal.B. Cover Letter: The response should contain a Cover Letter submitted on the Proposer’s official businessletterhead and signed by a person who is authorized to commit to the Proposer to perform the work /services included in the proposal and should identify all materials and enclosures being forwarded inresponse to the RFP. By signing the Cover Letter and the Proposal, the Proposer certifies compliance withthe signature authority required in accordance with LA R.S. 38:2212 B (5) as may be amended.C. General Information: Brief discussion about Proposer’s business history and current purpose / function inthe marketplace/community.RFP: 25-PLAN-48 Contract for Grass Cutting of Offensive Accumulations and Trash RemovalPage 3D. Qualifications & Experience: The Proposer should demonstrate qualifications and experience by includingthe following:• A statement of the Proposer’s understanding of the requirements of the RFP• Proposer’s ability to meet the TPCG’s objectives and to perform the tasks and/or the servicesprovided in the RFP• The Proposer must hold and maintain one of the following licenses: Landscaping Grading andBeautification (Louisiana Contractors Licensing Board) or Landscape Horticulturist (LouisianaDepartment of Agriculture and Forestry). A copy of the Proposer’s license MUST be submitted withthe proposal.• The Proposer must have been in business for a minimum of two (2) years. The Proposer shallpresent documentation verifying that experience.• The Proposer shall maintain offices, shop facilities and personnel located in Terrebonne Parish.• The Proposer shall be accessible by email and telephone from Terrebonne Parish during regularbusiness hours.• The Proposer shall submit a list of names of their managers and/or supervisors.• Provide at least three (3) references. Include the name of the organization, the length of contract(if applicable), a brief summary of the work and the name and telephone number of the personresponsible for overseeing the contract.• Proposer must be the prime Contractor for each customer/contract referenced. All referencesshall be for work performed over the last twelve (12) months at commercial, multi-residentialdevelopments and/or institutional complexes for contracts of comparable size.• At least one (1) contract/customer shall have been serviced for a minimum of one (1) year. Failureto provide verifiable references may result in the Proposer not being considered for award.• Unsatisfactory references may result in the Proposer not being considered for award.E. Project Approach: Proposer should include at a minimum:• A management and response plan applicable for the scope of work• List of personnel who will be involved in the management of this Agreement. Include theirexperience in the area of service delivery. Indicate the level of involvement they will have in theday-to-day operation of the Agreement.F. Price Proposal:• Proposer shall submit pricing on Price Proposal Form included in this RFP packet.G. Certificates of Insurance: Proposer should include certificates of insurance as required hereafterH. Attachments: Proposer must include in their proposal all documentation that will be used during the course ofthis Agreement. Proposer in all cases shall be in a position to assure a timely completion of services to the TPCG.Proposer will be asked to commit to an acceptable response and turn-around time as a performance parameterto this Agreement. Proposer will be audited during the Contract to confirm that performance commitments arebeing met.Additional information, which the Proposer feels will assist in the evaluation should be included.I. Conflict of Interest Disclosure: All Respondents providing a response to this RFP shall provide a clear andunambiguous indication of any potential or real conflicts of interest it may have with respect to performingwork on behalf of TPCG. TPCG shall make the final determination as to whether any potential or realconflict of interest exists. Please provide in writing whether or not a conflict exists.J. Litigation: The Respondent should provide a list of any previous, ongoing, or pending litigation orarbitration in which the Respondent’s firm has been involved during the past five (5) years with respect tothe provision of these or similar services.RFP: 25-PLAN-48 Contract for Grass Cutting of Offensive Accumulations and Trash RemovalPage 42.3 Number of Response CopiesEach Proposer shall submit one (1) bound signed original response and one (1) electronic copy submitted in PDFformat on a thumb drive and one (1) redacted copy, if applicable.NOTE: Proposal must be signed by an individual that has the authority to enter into a contractual agreement andevidence of authority must be included with proposal submission.2.4 Legibility / ClarityResponses to the requirements of this RFP in the formats requested are desirable with all questions answered in asmuch detail as practicable. The Proposer’s response is to demonstrate an understanding of the requirements.Proposals prepared simply and economically, providing a straightforward, concise description of the Proposer’sability to meet the requirements of the RFP is also desired. Each Proposer is solely responsible for the accuracy andcompleteness of its proposal.2.5 Confidential Information, Trade Secrets and Propriety InformationThe designation of certain information as trade secrets and/or privileged or confidential proprietary informationshall only apply to the technical portion of the proposal. The cost proposal will not be considered confidentialunder any circumstance. Any proposal copyrighted or marked as confidential or proprietary in its entirety may berejected without further consideration or recourse.For the purposes of this procurement, the provisions of the Louisiana Public Records Act (La. R.S. 44.1 et. seq.) willbe in effect. Pursuant to this Act, all proceedings, records, contracts, and other public documents relating to thisprocurement shall be open to public inspection. Proposers are reminded that while trade secrets and otherproprietary information they submit in conjunction with this procurement may not be subject to public disclosure,protections must be claimed by the Proposer at the time of submission of its Technical Proposal. Proposers shouldrefer to the Louisiana Public Records Act for further clarification.The TPCG reserves the right to make any proposal, including proprietary information contained therein, availableto TPCG personnel or organizations for the sole purpose of assisting the TPCG in its evaluation of the proposal. TheTPCG shall require said individuals to protect the confidentiality of any specifically identified proprietary informationor privileged business information obtained as a result of their participation.Proposer’s failure to comply with La. R.S. 44:3.2. (D)(1) shall result in the failure to properly assert the designationof trade secrets and/or privileged or confidential proprietary information and the information may be consideredpublic records.If your proposal contains confidential information, you should submit a redacted copy along with your proposal. Ifyou do not submit the redacted copy, you will be required to submit such a copy within forty-eight (48) hours ofnotification from the Purchasing Division, if you intend to maintain confidentiality. When submitting your redactedcopy, you should clearly mark the cover as such - “REDACTED COPY” - to avoid having this copy reviewed by anevaluation committee member.
