Evidence-Based Wellness Services:
The Middle Georgia Regional Commission (MGRC) is soliciting proposals from potential Offerors interested in providing Evidence Based Wellness services for the following counties: Baldwin, Crawford, Houston, Jones, Macon-Bibb, Monroe, Peach, Pulaski, Putnam, Twiggs, and Wilkinson from July 1, 2026 – June 30, 2028.Proposals should be submitted to Kim Stanton no later than December 22, 2025, 4:00 PM EST.
Please submit one original and four copies of the completed bid proposal. Bids must be packaged, submitted, and received by the Middle Georgia Regional Commission by the due date and due time noted above. Late submissions will not be accepted.

2027-20281.1 Purpose of ProcurementThrough this Request for Proposal (RFP), the Middle Georgia Regional Commission (MGRC) issoliciting proposals from potential Offerors interested in providing Evidence Based Wellnessservices for the following counties: Baldwin, Crawford, Houston, Jones, Macon-Bibb, Monroe,Peach, Pulaski, Putnam, Twiggs, and Wilkinson from July 1, 2026 – June 30, 2028.The Middle Georgia Regional Commission will accept competitive bids to provide one or more ofthe following services:Evidence-Based Wellness ServicesThe Middle Georgia Regional Commission may award contracts to service providers in each ofthe counties, any combination of counties, or for the region as a whole. The scope of servicesprovided in the appendices indicates which services must be bid on as a region. The MiddleGeorgia Regional Commission has the right to reject any and all Offerors.1.2 Vision, Mission, and Values of the Middle Georgia Area Agency on Aging• Vision: The Middle Georgia Area Agency on Aging is the local Aging services leader of acustomer-driven Aging Services Network known for its exemplary teamwork, quality,innovation, and accountability.• Mission: The mission of the Middle Georgia Area Agency on Aging is to provide assistance inthe development of new or improved programs to help older persons and their caregiversthrough grants for community planning, services, and training, and the development of acomprehensive service delivery system with a continuum of care to assist older MiddleGeorgians and their caregivers in achieving healthy, independent, and self-sufficient lives.• Values: A strong customer focus, accountability and results, teamwork, dignity, andempowerment.1.3 Overview of the RFP ProcessThe objective of this RFP is to select qualified Offerors to provide the services as outlined in theRFP on behalf of the Middle Georgia Regional Commission. This RFP process will be conductedto gather and evaluate responses from Offerors for potential awards. All qualified Offerors areinvited to participate by submitting responses, as further defined herein. After evaluation, allresponses received prior to the closing date of this RFP and following negotiations (if any) andresolution of any contract exceptions, the preliminary results of the RFP process will be publiclyannounced, including the names of all participating Offerors and evaluation results.4REQUEST FOR PROPOSALMiddle Georgia Regional CommissionSFY 2027-2028Offerors must complete the Request for Proposal and Letter of Transmittal to be considered. TheMiddle Georgia Regional Commission has the right to reject any and all proposals or to waive anytechnicalities. If there is more than one qualified Offeror for a specific service, a final award willbe based on the Middle Georgia Regional Commission’s review and evaluation of the bidsreceived. Selected Offerors will become a part of the service delivery system detailed in theMiddle Georgia Regional Commission’s FY 2027-2028 Area Plan on Aging Services to be submittedto the Georgia Division of Aging Services no later than March 1, 2026. Inclusion in the Area Plandoes not guarantee or imply any grant award for subsequent years.1.4 Letter Of Intent RequirementsEntities planning to respond to this RFP must submit a Letter of Intent (LOI) to kstanton@mg-rc.org by November 17, 2025, with the RFP in the subject line of the email. The LOI may besubmitted as an attachment or included in the body of an email and must include theorganization’s name, point person name, and contact information. In addition, the LOI mustindicate the county and service for which the Offeror intends to submit. If the LOI is not received,proposals will be automatically disqualified. If no one submits a LOI for a service, MGRC reservesthe right to extend the procurement process for that service by issuing an addendum.1.5 Schedule of EventsThe schedule of events outlined herein represents the Middle Georgia Regional Commissionsbest estimate of the schedule that will be followed. However, delays to the procurement processcould occur that may necessitate adjustments to the proposed schedule. If a component of thisschedule, such as the close date, is delayed, the remainder of the schedule may be shifted asappropriate. Any changes to the dates up to the closing date of the RFP will be publicly postedat http://www.middlegeorgiarc.org/ before the closing date of this RFP.
