| RFP ,DHS is seeking to prevent or end homelessness for people with serious mental illness or co-occurring substance use disorder, to increase the availability of housing with support, and to achieve the goals of the housing mission statement.DHS, through its Behavioral Health Division, is seeking to provide Projects for Assistance in Transition from Homelessness (PATH) homeless outreach services, and Housing with Supports for Adults with Serious Mental Illness grant program (HSASMI) housing services, for adults with serious mental illness, or co-occurring SMI and substance use disorder who are homeless or at imminent risk of homelessness, have complex needs, and face high barriers to obtaining and maintaining housing.The anticipated term of any resulting contract and expected funding to be available are:1. PATH – Term January 1, 2025, until June 30, 2026, for up to 18 months. Total program funding of $875,000 for State Fiscal Year (SFY) 25 (January 1, 2025 to June 30, 2025) and$1,670,000 for SFY26 (July 1, 2025 to June 30, 2026).2. HSASMI – Term January 1, 2025, until June 30, 2026, for 18 months. Total program funding of $51,000 for SFY25 (January 1, 2025 – June 30, 2025) and $39,000 for SFY26 (July 1, 2025 – June 30, 2026). - Commodity Codes
