The following revisions are hereby made to the Contract Documents for the above-referenced project. All

bidders shall acknowledge receipt of this Addendum on the proposal form and shall incorporate all changes

into their submitted bid. Failure to acknowledge receipt of this Addendum may result in disqualification of the

bid.

1. Proposal Form (Page 8)

• A revised Proposal Form (page 8) is attached. This revised Proposal Form must be used when

submitting the bid. Use of prior versions may result in rejection of the bid.

2. Specifications-Scope of Work (Page 11)

• Change oil-based paint to latex paint.

3. Pre-Bid Documentation

• The Pre-Bid Sign-In Sheet is attached for reference.

All other terms and conditions of the original RFP remain unchanged.
