Category: Professional Services Status: Open Description: Base Bid – “North Donnelly St Emergency Roadway Repair (Permanent"): This is a roadway reconstruction project along Donnelly Street from approximately 120 feet north of Limit Avenue to approximately 180 feet south of the existing pedestrian bridge. The project also includes the installation of a 24” culvert and U-endwalls with baffles. The total length of the project is approximately 1,100 linear feet. This project includes the reconstruction of Donnelly Street including milling and resurfacing, guardrail installation and striping. Other construction elements include mobilization, erosion control, clearing and grubbing, regular excavation, embankment, stabilization and sod. Bid Alternate No. 1 – “North Donnelly Street Slope Stabilization, Erosion Control and Sidewalk Improvements.”: This work consists of the construction of roadway embankment improvements on a portion of North Donnelly Street north of Limit Avenue to improve stormwater drainage improvements and pedestrian safety. Project items include removal of existing pedestrian boardwalk and drainage inlets and piping, and widening the roadway to include a 4’ wide asphalt bike lane, approximately 1,100 linear feet of concrete curb and gutter, 10’ wide concrete pedestrian trail with railing, three pedestrian shelters with benches, stormwater inlets, piping with outlet structures, and additional embankment with landscaping.

The Contractor’s Engineer of Record may also serveas the Specialty Engineer.The Contractor’s Engineer of Record must be an employee of a pre-qualified firm. Thefirm shall be pre-qualified in accordance with the Rules of the Department of Transportation,Chapter 14-75. Any Corporation or Partnership offering engineering services must hold aCertificate of Authorization from the Florida Department of Business and ProfessionalRegulation.As an alternate to being an employee of a pre-qualified firm, the Contractor’s Engineer ofRecord may be a pre-qualified Specialty Engineer. The education and experience necessary to perform the submitted design asrequired by the Florida Department of Business and Professional Regulation.-4-FPID(S): 457929-2-58-01SECTION 2 – PROPOSAL REQUIREMENTS AND CONDITIONS2-1 Prequalification of Bidders.Except as noted below, prequalify with the Department to be eligible to bid. TheDepartment publishes regulations covering prequalification of Bidders under separate cover.The Department does not require the Bidder to be a prequalified Contractor if biddingconstruction contracts of $250,000 or less, or if constructing buildings. In addition, at its solediscretion, the Department may waive prequalification requirements on contracts of $500,000 orless.For construction contracts requiring prequalification, file an application for qualificationusing the Department’s online prequalification application system, giving detailed informationwith respect to financial resources, equipment, past record, personnel, and experience. Forqualified applicants, the Department will issue a certificate fixing the types of work and theaggregate amount of work that the Department allows the prequalified Bidder to have undercontract at any one time.A person or affiliate who has been placed on the convicted vendor list following aconviction for a public entity crime may not submit the following:1. A bid on a Contract to provide any goods or services to a public entity.2. A bid on a Contract with a public entity for the construction or repair of apublic building or public work.3. Bids on leases of real property to a public entity.A person or affiliate who has been placed on the convicted vendor list following aconviction for a public entity crime may not be awarded or perform work as a contractor,supplier, subcontractor, or consultant under a contract with any public entity, and may nottransact business with any public entity in excess of the threshold amount provided inSection 287.017 F.S., for Category Two. All restrictions apply for a period of 36 months fromthe date of placement on the convicted vendor list.All prequalified Contractors bidding on any Contract must certify their total dollaramount of Work Underway and submit Form 375-020-39 or a spreadsheet in a similar formatprior to submitting a bid. This information must be submitted at least once during the month thebid is due via the “Work Underway” link in the Contractor Pre-Qualification System.SECTION 4 – SCOPE OF THE WORK.4-3 Alteration of Plans or of Character of Work.4-3.1 General: The Engineer reserves the right to make, at any time prior to or during theprogress of the work, such increases or decreases in quantities, whether a significant change ornot, and such alterations in the details of construction, whether a substantial change or not,including but not limited to alterations in the grade or alignment of the road or structure or both,as may be found necessary or desirable by the Engineer. Such increases, decreases or alterationsshall not constitute a breach of Contract, shall not invalidate the Contract, nor release the Suretyfrom any liability arising out of this Contract or the Surety bond.
