INVITATION FOR BID FOR FEMA REPAIRS
PROCUREMENT INFORMATION
The Housing Authority of The City of Live Oak’s FEMA Repairs Committee will accept Sealed Bids from
licensed and insured contractors to fix FEMA Repairs.
Interested contractors shall submit one (1) original signature copy (marked “ORIGINAL”) with two (2) exact
copies with your bid submission. Bids should be marked “FEMA REPAIRS” and must be submitted to the
following address no later than 4:00 PM on June 30, 2026___.

All bids shall be time and date stamped upon receipt. Late bids will not be accepted. All bids received will
become a part of The Housing Authority of The City of Live Oak’s official files without any obligation on The
Housing Authority of The City of Live Oak’s part to return submitted material.
Bidders will provide all necessary permits, labor, materials, tools, equipment, fuel, transportation, lodging (if
applicable) and supervision necessary to complete scope of work.
Bidders may access the property to review the FEMA REPAIRS during business hours of the LOHA, Monday –
Thursday (8:00 am – 5:00 pm) EST (except federal holidays as the office is closed) and any additional mutually
agreed upon times between Bidder and Executive Director on as needed basis. Bidders will check in with the
LOHA office upon arrival at the property and check out when leaving property.
All BIDS received in accordance with the Invitation for BID will be reviewed and evaluated by the Housing
Authority. Bidders will then be ranked based on their evaluation of the best-qualified, experienced, bid price
and responsive Bidder.
Live Oak Housing Authority reserves the right to reject any or all Bids and to select the Bidder which, in its
judgement, best meets the needs of Live Oak Housing Authority.
BACKGROUND
Live Oak Housing Authority (LOHA) has 104 low rent units, 2 playgrounds, ½ acre vacant land, and
approximately 1 acre pond with grass area surrounding it.

Project Notes:
1. In accordance with FEMA’s 422 Simplified Procedures, the project scope and cost were developed by FEMA’s
CRC from the applicant signed DDD. The project was certified by the Applicant using the Small Permanent Work
Project Certification and Acknowledgements Form located in the Essential Elements of Information (EEI). See

2. All site estimates for work to be completed were generated using RS Means. See attachment labeled:

3. Applicant will comply with its local, state, federal procurement laws, regulations, and procedures.
4. For disasters declared on December 20, 2019, or newer, applicants are required to follow the provision of
Consensus Based Codes, Specifications and Standards for Public Assistance FEMA Recovery Interim Policy FP-104-
009-11 Version 2.1 for permanent work. For disasters declared between August 1, 2017, and December 20, 2019,
applicants may “opt-in” to the requirements of the policy by summiting the opt-in notification form found in the
policy. Requirements, implementation, Identification requirement, verification requirements, additional eligible
work, and cost criteria as well as the Appendix A table which lists Consensus Based Codes, Specification and
Standards by facility type can be found in policy FP-104-009-11.
5. Project declaration date is after 1/6/2023, In accordance with FEMA’s Simplified Procedures policy, FEMA CRC
developed the Scope and Cost with the information/documentation/certification provided by the Applicant.
6. All borrow or fill material must come from pre-existing stockpiles, material reclaimed from maintained
roadside ditches (provided the designed width or depth of the ditch is not increased), or commercially procured
material from a source existing prior to the event. For any FEMA-funded project requiring the use of a non-
commercial source or a commercial source that was not permitted to operate prior to the event (e.g. a new pit,
agricultural fields, road ROWs, etc.) in whole or in part, regardless of cost, the Applicant must notify FEMA and
the Recipient prior to extracting material. FEMA must review the source for compliance with all applicable federal
environmental planning and historic preservation laws and executive orders prior to a subrecipient or their
contractor commencing borrow extraction. Consultation and regulatory permitting may be required. Non-
compliance with this requirement may jeopardize receipt of federal finding. Documentation of borrow sources
utilized is required at closeout.
7. All demolition debris must be disposed of at a permitted landfill. Documentation of demolition disposal shall
be provided at project close-out.
1501944 Harmony Triangle Live Oak Housing Authority Sidewalk Repairs
Work to be Completed:
The applicant will utilize contract and (or) force account for repairs to the Harmony Triangle Live Oak Housing
Authority Sidewalk, to restore this facility to its pre-disaster design, function, and capacity (in-kind) within the
existing footprint.
A. Replace 1 EA of Concrete Sidewalk Pad, 5 FT Long x 4 FT Wide x 3 IN Deep.
B. Replace 4 EA of Concrete Sidewalk Pad, 5 FT Long x 5 FT Wide x 3 IN Deep.

Authority Playground Top Surface, to restore this facility to its pre-disaster design, function, and capacity (in-kind)
within the existing footprint.
A. Replace 55.5556 CY of Sand Fill, 60 FT Long x 50 FT Wide x 6 IN Deep.
