Our answers are listedbelow in red:1. Page 32, Cost and Fees, 4.a: the RFP says, “If multi-year, include a breakdownper fiscal year (July 1st to June 30th) for each renewal year. My question: thegrant year is March 1 to February 28 (or 29). Does an adjustment to fit theShelby County fiscal year have to be made or should I use the grant’s fiscalyear? Please use the grant year March 1st – February 28th (or 29th), we’ll issue anaddendum to align with the grant year.2. On page 33, References: the RFP asks for “At least three (3) current or formerclients who have terminated in the last five (5) years, are to be included” in a listof current and former clients. Does the RFP want names and contact informationfor vendors our agency has worked with or is the RFP asking for the names ofclients who have received mental health services with our agency? My concern isthat if the latter is what the RFP is requesting, this would be HIPAA protectedclient information and assurances of confidentiality are needed. Would youplease clarify exactly what information is requested?References, names and contact information for vendors your agency has workedwith to provide services similar to this RFP. We don’t want names of clients whohave received mental health services.3. On page 4, II Minimum Proposer Requirements2. BRN #, does this apply to Crittenden County Arkansas bidder? The Proposal Response Sheet also has the BRN# as a requirement. What would be the response if not appliable to Arkansas? As a part of doing business with Shelby County Government (SCG), eachindividual, company, or organization is required to have active BRNL and Vendornumbers before starting to provide goods/services to SCG.• To obtain an SCG Equal Opportunity Compliance (EOC) BRN (as well asan BRNL, if applicable) and the SCG Vendor number, please submit anonline application, as described below.
