1. Revised Bid Form for Construction Contract to incorporate the clarifications from

previous Addenda, 1 through 3.

This addendum revises the “Bid Form for Construction Contract”. See below, the next

pages.

NOTE: Items 0406171, 0406172, 0406236, 0406286, 0803010, 0815001, 1210102, 1210106

from Addendum #2, paragraph 2. that were clarified being done by S&S Asphalt Paving

Inc., are shown in this revised Bid Form with strikethrough lines in order not to be filled in.
