ARCHER SEPTIC IMPROVEMENTS 2026

ACHA will not be able to accept your quote via email as originally noted in the Request for Quote.
You may deliver your sealed quote with required attachments personally or via a delivery service like USPS, FedEx, UPS, etc. However, the Alachua County Housing Authority is not responsible for delays in the delivery of your submission regardless of the form of delivery.

Terms and Conditions3.1 The Awardee agrees to comply with the terms and conditionsas specified in documents and forms as Attached:3.1.1 From HUD 5370-EZ General Conditions for SmallConstruction/Development Contracts ($2,000 and$250,000)3.1.2 Davis Bacon Wage Rate Determination GeneralDecision Number FL20260052 01/02/2026 MOD 03.1.2.1Please note that verification of wages will berequested through employee interviews and/oradditional backup documentation3.1.3 Legally Required Statement and Provisions RegardingAccess to Records4. Special Conditions4.1 Inspection ACHA: The Director of Maintenance will inspect allwork prior to payment for work completed to date of request.4.2 Inspection Alachua County Florida: Permit inspections arerequired to be completed in person. There will be no self-certification of inspections. A satisfactory approval ofcompleted permit must accompany work completed to date ofrequest.4.3 Change Orders: Your quote should address all items necessaryto complete the Scope of Work. However, ACHA understandsthat there may be items that are discovered while completingthe requested work. These will be addressed as they arise.ACHA may make Change Orders adding or deleting work asnecessary if it is in the best interest of the Authority to do so.Both parties shall make no modifications of the terms of thecontract except by written amendment or change order.Contractor shall not perform extra work unless authorized todo so in writing. Contractor needs to submit a bid that takesinto account all aspects of the request. Failure to include costsfor items that should have been in your bid will may not bereimbursable. I.E. Do not low bid with the expectation to winthe bid, but then secure a change order.4.4 Invoicing/Payment: Pay requests are be sent to Ron Hall viaemail at Ron@acha-fl.com for work completed and inspectedto date. Along with the invoice, please include Certified Payroll,a summary of units completed to date, and approvals of workcompleted by both the Director of Operations and the FloridaDepartment of Health in Alachua County.REQUEST FOR QUOTE 3RFQ Archer Septic Improvements 20264.5 ACHA understands that this procurement entails a very largeoutlay of upfront costs to the awardee for equipment. ACHAhas a process that allows for payment on Materials Stored.4.6 Sub-Contracting: Contractor agrees not to hire any sub-contractor without prior approval and clearance. Any sub-contractor associated with a potential award must follow andadhere to the same procurement requirements including thesubmission of required forms as the prime contractor. Sub-contractors must be listed in your response along with thesame required forms as the prime contractor.4.7 Insurance: Contractor agrees to provide proof of Insurance asfollows:4.7.1 Automobile Liability Insurance on owned and non-owned motorvehicles used in connection with this contract and shall contain alimit of bodily injury of at least $500,000 per person and a totallimit per occurrence of at least $1,000,000 and Property DamageLimit of $500,000 per accident.4.7.2 Commercial General Liability Insurance with a combined singlelimit for bodily injury of not less than $1,000,000 per occurrenceand $500,000 property damage per occurrence to protect theContractor and the Authority against claim for bodily injury ordeath and damage to the property of others4.7.3 Worker’s Compensation Insurance shall be maintained inaccordance with Section 440.185(4), Florida Statutes. Statutoryrequirements and benefits. Coverage is compulsory foremployers of three or more employees, to include the employer.Contractors who fail to notify ACHA of increases in the numberof employees that changes their workers’ compensationrequirements under the Florida Statutes during the course of thecontract shall be in noncompliance with the contract.4.7.4 ACHA shall be named as “Additionally Insured” and “CertificateHolder” on all policies.4.8 Licensing: Contractor shall provide ACHA with a copy of its current BusinessLicense, any licensing as required for the trade and FEIN#. When necessary,contractor shall make all necessary applications for permits and/or licenseswhich are required for the execution of the work.4.9 Section 3 Economic Opportunities: The work to be performed under thiscontract is subject to the requirements of Section 3 of the Housing andUrban Development Act of 1968, as amended, 12 W.S.C. 170lu (Section 3).The purpose of Section 3 is to ensure that employment and other economicopportunities generated by HUD assistance or HUD-assisted projectscovered by Section 3 shall, to the greatest extent feasible, be directed tolow and very low-income persons, particularly persons who are recipients ofREQUEST FOR QUOTE 4RFQ Archer Septic Improvements 2026HUD assistance for housing.4.9.1 The Contractor is responsible for making every effort to ensurethat employees from ACHA Section 3 residents have first right ofrefusal when hiring new employees under this contract.4.9.2 The parties to this contract agree to comply with HUD’sregulations in 234 CFR part 135, which implement Section 3. Asevidenced by the execution of this contract, the parties to thiscontract certify that they are under no contractual or otherimpediment that would prevent them from complying with thepart 135 regulations.4.9.3 The contractor agrees to send to each labor organization orrepresentative of workers with which the contractor has acollective bargaining agreement or other understanding, if any, anotice advising the labor organization or workers’ representativeof the contractor’s commitments under this Section 3 clause, andwill post copies of the notice in conspicuous places at the worksite where both employees and applicants for training andemployment positions can see the notice. The notice shalldescribe the Section 3 preference, shall set forth minimumnumber and job titles subject to hire, availability ofapprenticeship and training positions, the qualifications for each;and the name and location of the person(s) taking applicationsfor each of the positions; and the anticipated date the work shallbegin.4.9.4 The contractor agrees to include this Section 3 clause in everysubcontract subject to compliance with regulations in 24 CFRpart 135, and agrees to take appropriate actions, as provided inan applicable provision of the subcontract or in this Section 3clause, upon a finding that the subcontractor is in violation of theregulations in 24 CFR part 135. The contractor will notsubcontract with any subcontractor where the contractor hasnotice or knowledge that the subcontractor has been found inviolation of the regulations in 24 CFPR part 135.4.9.5 The contractor will certify that any vacant employment positions,including training positions, that are filled: 1) after the contractoris selected but before the contract is executed, and 2) withpersons other than those to whom the regulations of 24 CFR part135 require employment opportunities to be directed, were notfilled to circumvent the contractor’s obligations under 24 CFPpart 135.4.9.6 Noncompliance with HUD’s regulations in 24 CFR part 135 mayresult in sanctions, termination of this contract for default, anddebarment or suspension from future HUD assisted contracts.REQUEST FOR QUOTE 5
