TO ALL PROSPECTIVE BIDDERS:

The following questions were submitted by potential bidders. Our answers are listed

below in red:

1. Is this RFP looking for eye care providers?

No, Eye care is not funded under core medical or supportive services under this

grant.

2. Are we allowed to revise these (budget) forms to meet our needs per program,

such as add line items not currently on the form?

Yes, the budget forms can be revised to meet the needs of agencies.

3. It is my assumption that, as in the past, each program is to have its own budget.

Is this correct?

Yes, please provide a budget for each category that the agency would like to be

funded for.

4. My question is, is this the page (checklist) that requires the company letterhead

or is it the page that is entitled: Proposal Response Sheet? Or, do both need to

be on company letterhead?

The page titled Proposal Response Sheet is the one required to be printed on

company letterhead.

5. Would it be possible for the RFP to be revised to make the watermark smaller

and lighter?

We’re unable to revise the RFP as requested.
