The bidpacket should include (in this order):• Bid Summary (attached in this packet)• Acknowledgement of bid amendments, if any• Itemized quote• Product and labor warranty• Any other information deemed necessary by bidder to aid in award of RFPIf selected, Vendor, unless already on file, will need to provide the followinginformation:• Certificate of Insurance• E-Verify Affidavit• Current W-9RFP CostAll costs incurred in the preparation and presentation of responses to the RFP shall becompletely absorbed by the respondent. All documents submitted as part of the RFP willbecome property of Morgan County. Fax and e-mail responses are not acceptable. Responses to the RFP will be accepted until MondayDecember 22, 2025, at 10:30 a.m. Bids will be opened at that time at the Board ofCommissioners Office located at 150 E. Washington St. Madison, GA 30650Responses must be delivered to:Mr. Mark Williams, Assistant County ManagerMorgan County Board of Commissioners150 E. Washington StreetMadison, Georgia 30650Requests for extending this date will not be granted. Any late proposal will not beconsidered. It is the sole responsibility of the Respondent to ensure that the proposalarrives at the designated location prior to the deadline.Morgan County reserves the right to reject any and all proposals, to waive anyinformality or technicality and to accept that proposal which, in the judgement of theproper officials, is in the best interest of Morgan County.Prebid MeetingThere will be a mandatory pre-bid meeting on Wednesday December 3, 2025, at 10:30a.m. The pre-bid meeting will be held at the Morgan County Health Department locatedat 2005 S. Main Street, Madison, GA 30650. Answers to questions submitted will be communicated torespondents via email.RFP AmendmentsIt is the responsibility of respondents to check the Morgan County website for RFPinformation and amendments.Non-CollusionBy submitting a proposal, the respondent represents and warrants that no official oremployee of Morgan County has an interest, directly or indirectly, in the proposal or inthe final contract award. The respondent further understands proposals will be rejected ifthere is any evidence of collusion with another respondent.Compliance with LawsRespondents agree to be bound by applicable Federal, State and Local laws, regulations,and directives.Qualification & Award BasisMorgan County reserves the right to accept or reject any and all proposals or any parts ofa proposal wherein its judgment, it will be in the best interest of the County; waive anytechnicalities/informalities in the RFP document and proposal process; and to qualify andaward any or all of this contract in any manner in which Morgan County, acting in thesole and exclusive exercise of its discretion, deems to be in Morgan County’s bestinterest. Cost will not be the sole factor in the selection process. If no acceptableproposal is received Morgan County also reserves the right to re-solicit proposals, at itssole discretion.Page 3 of 5Required Bonds and InsuranceA certificate of insurance with minimum coverage limits of $1,000,000 general liability,$150,000 automobile liability and statutory limits for worker’s compensation must besubmitted by awarded bidder before work may begin.Completion InspectionMorgan will periodically conduct inspections during construction. A final inspection willbe conducted before accepting the project as complete and processing final payment.TrainingThe successful bidder shall provide training for care and maintenance of all installedproducts and equipment.SERVICES REQUESTEDPurpose of RequestMorgan County Board of Commissioners are seeking bids for the replacement of flooringat the Health Department. The specifications below address the specific requirementstogether with certain details as to finish, material preferences, equipment and applianceswith which the successful bidder must conform. The bidder shall bear the responsibilityof insuring that the elements contained within the specifications are sound, safetyconscious, and functional.WarrantyAll bidders shall state the length and terms of warranty for all applicable warranties.State Delivery TimeThe bidder shall estimate the expected delivery date from time of order.Page 4 of 5SCOPE OF WORKBidder responsible for all labor and materials to successfully complete the project• Remove and dispose of all existing flooring materials.o Note: The County will provide tipping at the Morgan County TransferStation for no fee.• Prepare the subfloor to ensure it meets manufacturer’s standard for installation ofthe new floor.• Move furniture out of the way to install new floor and return to original locationafter the new floor is installed.• Product: Revolution Mills – Aspire Collection – Color: To Be Determined (TBD).• Installation Areas: The installation areas will be identified at the pre-bid meeting.Bidder is responsible for measuring the area to determine the amount of materialneeded.• Install new cove base (color to be determined)• Perform a thorough final clean-up of all work areas, removing all tools, excessmaterials, and construction debris.Evaluation ProceduresAn evaluation team will evaluate proposals submitted. Each proposal will be evaluatedfor full compliance with the RFP instructions to the Respondent and the requirements setforth within the RFP document. Proposals will be evaluated on criteria will include• Compliance with specifications detailed in RFP (50%)• Warranties included (30%)• Estimated completion date (15%)• Price (5%)Page 5 of 5
