Project Description: Issuing Procurement Unit: State of Utah Division of Purchasing
Conducting Procurement Unit: Department of Health and Human Services
The purpose of this RFP is to enter into a contract to provide: Locum Tenens Services for pharmacist staffing on an as-needed basis, as requested and directed by the DHHS.
It is anticipated that this RFP will result in multiple contract awards.
The methodology or formula that will be used to determine the number of contract awards is the vendors that receive a minimum score threshold of 420 Technical Points and a combined threshold of (Technical points + Cost Points) of 600 points will receive a contract award.
Length of Contract: Five Years.
Vendor must guarantee its pricing for One Year.
