The Proposal should demonstrate the qualifications of the Respondentand of the particular staff to be assigned to this project.1. INTRODUCTION1.1 About the CityThe City of Aliso Viejo was incorporated July 1, 2001, as a general law citywhich, operates under the council/manager form of government. The City encompassesapproximately seven (7) square miles; serves an approximate population of 50,000 people andis located in southern Orange County.The City provides municipal services primarily through contracts for police andfire services, public works, engineering, building, code enforcement, recreation, and legalservices. The City’s fiscal year begins on July 1 and ends on June 30.The Aliso Viejo Public Facilities Corporation was established in July 2006 for thepurpose of providing lease financing to the City for the acquisition of an existing office buildingto serve as the City Hall facility.1.2 Project DescriptionThe City is looking for a consultant capable of performing geotechnical or materialstesting services to ensure construction complies with the contract documents. Anticipated Final Selection6. Project Start4/9/264/22/264/29/26N/A6/3/267/1/263. SCOPE OF REQUIRED SERVICES3.1 Scope of WorkA detailed Scope of Work is attached hereto as Attachment "1". The Respondentwill be expected to fulfill, at a minimum, the services and technical requirements described inthe attached Scope of Work. Upon final selection, the Scope of Work may be modified andrefined during negotiations with the City.3.2 Prevailing Wage; Public Works Contractor RegistrationCertain labor categories under this project may be subject to prevailing wages asidentified in the State of California Labor Code commencing at sections 1720 et seq. and 1770et seq. If applicable, employees working in these categories at the site must be paid not lessthan the basic hourly rates of pay and fringe benefits established by the California Departmentof Industrial Relations (“DIR”). Copies of the State of California wage schedules are available forreview at www.dir.ca.gov/dlsr/. In addition, a copy of the prevailing rate of per diem wages willbe made available at the City upon request. The successful Respondent shall post a copy ofthe prevailing wage rates at each job site. It shall be mandatory upon the successfulRespondent, and upon any subcontractors, to comply with all Labor Code provisions, whichinclude but are not limited to the payment of not less than the said specified prevailing wagerates to all workers employed by them in the execution of the Agreement, employment ofapprentices, hours of labor and debarment of contractors and subcontractors. The contractawarded pursuant to this RFP may also be subject to compliance monitoring and enforcementby the DIR.If the scope of services to be provided are subject to prevailing wages as definedby Labor Code Section 1720, et seq. and 1770, et seq., then pursuant to Labor Code sections1725.5 and 1771.1, all contractors and subcontractors that wish to bid on, be listed in a bidproposal, or enter into a contract to perform public work must be registered with the Department60367.01100\33489632.1City of Aliso Viejo-2-Request for Proposalsof Industrial Relations. No proposal will be accepted nor any contract entered into without proofof the contractor's and subcontractors' current registration with the Department of IndustrialRelations to perform public work. If awarded a contract, the successful Respondent and itssubcontractors, of any tier, shall maintain active registration with the Department of IndustrialRelations for the duration of the project. Notwithstanding the foregoing, the contractorregistration requirements mandated by Labor Code sections 1725.5 and 1771.1 shall not applyto work performed on a public works project that is exempt pursuant to the small projectexemption specified in Labor Code sections 1725.5 and 1771.1.4. PROPOSAL REQUIREMENTS4.1 Proposal ContentRespondent's Proposal shall be clear, accurate, and comprehensive. Excessiveor irrelevant materials will not be favorably received. The content and sequence of theinformation contained in each copy of the Proposal shall be as follows:4.1.1 Table of Contentsnumber.Include a clear identification of the material by section and by page4.1.2 Summary SheetInclude a fully-completed copy of the Summary Sheet included with thisRFP (Attachment 2, “Required Forms”).4.1.3 Cover LetterIdentify Respondent's legal name and contact information. Summarizekey elements of the Proposal. The letter must stipulate that the cost proposal will remain validfor a period of at least 180 days. The letter shall be signed by an individual or individualsauthorized to execute legal documents on behalf of the Respondent.4.1.4 Personnel.Identify the names and specific qualifications, experience, andappropriate licenses held, if applicable, of the primary staff to be assigned to the project.Include a resume for all primary staff. Identify any subconsultants which Respondent proposesto use for any portion of the services.Respondent shall also identify a Management Contact (Representativeauthorized to sign an agreement for your firm) and a Project Manager (person responsible forday-to-day management of the project). The successful Respondent may only change theManagement Contact, Project Manager, and other supporting staff and specialists with priorwritten permission of the City.4.1.5 Project ApproachProvide a conceptual plan for services to the City that you believe areappropriate for the City. Indicate features, skills and/or services which distinguish your firm andmake it the better choice for the City. Indicate how the resources of your firm (e.g., number and60367.01100\33489632.1City of Aliso Viejo-3-Request for Proposalstype of personnel allocated by hours) will be allocated for this project. Submit a project scheduleif applicable.4.1.6 Scope of WorkProvide a detailed scope of services which reflects the Scope of Servicesdescribed in this RFP. Proposals must address all items set forth in Section 3 "Scope of Work."Additional information which, in your opinion, should be included must be clearly identified.4.1.7 Experience(a) List of representative projects undertaken by Respondent in thelast 5 years demonstrating experience in each category of the project. Identify the names oforganizations, and names and telephone numbers of persons who can be contacted with regardto the services you have provided.(b) Identify all previous and current contracts with the City. Include acontact name, description of services, and dates of services performed.(c) List all public agencies (not to include the City) for which contractswere terminated in the last three years. Show names of organizations and names and telephonenumbers of persons who can be contacted. Firms may provide a brief explanation of thereason(s) for termination(s).4.1.8 Additional FormsThe following forms are included in Attachment “2” Required Forms:(a) Respondent must submit the attached “Acknowledgment ofInsurance Requirements” form acknowledging that it has reviewed the insurance requirementsand will provide such insurance. The applicable insurance requirements are set forth in thestandard agreement form, attached hereto as Attachment “3”.(b) Respondent must submit a Declaration of Political Contributions,signed under penalty of perjury, regarding disclosure of political contributions to City officials.Policy.(c) Respondent must submit the acknowledgement for the Fraud(d) If applicable, Respondent shall submit the Public WorksContractor Registration Certification.4.1.9 Cost ProposalInclude a detailed rate sheet inclusive of all hourly rates, anticipated travel, andother incidental costs and charges. The rate shall be submitted in a separate, sealed envelope,which shall be marked "Cost Proposal” and state Respondents name. If requested, thesuccessful Respondent may be required to submit an electronic copy of the cost proposal in MSWord, Excel or other format acceptable to the City.
