The work to be performed under this Professional Services Agreementincludes the furnishing of all labor, materials, tools, equipment, transportation, and incidentalsnecessary to complete the on-call masonry services.SECTION 2 – DISTRICT PROFILEThe District is located in California’s Central Valley in the southern portion of SacramentoCounty, four miles south of the Sacramento City limits. The District is the largest communityservices district in California encompassing 157 square-miles and serves an estimatedpopulation of 207,000.The District is a regional agency providing fire protection services to the cities of Elk Grove andGalt, as well as unincorporated areas in southern Sacramento County. Additionally, the Districtprovides parks and recreation services exclusively to the Elk Grove community. The District andCity of Elk Grove are independent agencies who work cooperatively for the benefit of thecommunity. The District departments include:x Administrative Services Department – Operations include execution and monitoring of theDistrict budget, long-range planning, information systems and the Facility andDevelopment which includes the planning, design, construction management andmaintenance for District property.x Fire Department – The Fire Department consists of two divisions, Operations andAdministration and Support Services, that work together to provide fire, rescue, andemergency medical services.x Parks and Recreation Department – Parks and Recreation manages 107 parks, 18 milesof off-street trails, two community centers, four recreation centers, three aquaticscomplexes, a nine-hole golf course and many recreation, sports, and leisure programs.The District is dedicated to enhancing the quality of life of the residents, businesses, visitors,and employees within the diverse community by protecting lives, property, and the environmentthrough superior fire suppression, emergency medical services, fire prevention, and specialoperations response; and by providing parks and recreation services through well-maintainedparks and recreational opportunities for health, wellness, and social interactions.SECTION 3 – SCOPE OF WORKThe scope of work can be found in Exhibit “A”.SECTION 4 – TERMS OF AGREEMENTLength of Service ContractThe initial term of the Professional Service Agreement (PSA) will be for a twelve (12) monthterm, from approximately January 16, 2026 through January 15, 2027. The District may1Cosumnes Community Services District RFPOn-Call Masonry Servicesextend the Agreement for up to two (2) additional one (1) year terms at the same prices, terms,and conditions.The cause and remedy for Contractor default or failure to perform satisfactorily will begoverned by Section 4 of the District’s PSA (Exhibit “D”).CompensationContractor shall provide the District with invoices for payment within 30 days of work beingcompleted.SelectionUpon selection, the Proposal and Contractor information will be presented to the District Boardof Directors for approval. The Contractor will then receive the PSA and will be expected toexecute the agreement within ten (10) business days of receipt. The Contractor shall providecompleted W-9, insurance requirements, and copies of certifications with signed PSA.Anticipated start date is January 16, 2026.x The price proposed is firm for the Scope of Work described in Exhibit “A” and cannot bealtered after receipt per the terms of this RFP.x Contractors will be required to obtain, and maintain for the duration of the contract,insurance as described below Section 5.SECTION 5 – MINIMUM QUALIFICATIONSThe Contractor has at least five (5) years of experience similar in nature and scope to theneeds of the District.InsuranceThe Contractor must provide evidence of commercial general liability, automobile liability, andproperty damage, professional liability and cyber liability insurance from an insurance companyauthorized to be in business in the State of California. See minimums listed below. All insurancepolicies shall contain a Waiver of Subrogation in favor of the Cosumnes Community ServicesDistrict. If Contractor will be employing any person or persons in the performance of this contract,Contractor shall procure a policy of Worker’s Compensation Insurance as required by the LaborCode of the State of California.Commercial General LiabilityAutomobile LiabilityWorkers Compensation$1,000,000 per occurrence for bodilyinjury, personal injury, and propertydamage / $4,000,000 aggregate.$1,000,000 per occurrence for bodilyinjury and property damage.$1,000,000 as required by the Stateof California, with Statutory Limits,and Employer’s Liability Insurancewith limit of no less than $1,000,000per accident for bodily injury ordisease.2Professional Liability (Errors &Omissions)Environmental InsuranceCosumnes Community Services District RFPOn-Call Masonry Services$1,000,000 per occurrence or claim /$2,000,000 aggregate.Not required for this solicitation dueto the repair-only scope; however, theDistrict reserves the right to mandatepollution/environmental liabilitycoverage on a project-specific basis ifwork later involves handlinghazardous materials, demolitiongenerating regulated dust, or theapplication of coatings or chemicals.Contractor will be required to provide an insurance certificate naming the District as additionallyinsured. The Additional Insured endorsement must be on a form at least as broad as ISO CG 2010 11 85 or equivalent. It must also state that coverage is primary and non-contributory withrespect to any insurance maintained by the District. Contractor must provide certificate showingactive policies of automobile liability, commercial general liability and property damage insurancefrom an insurance company authorized to be in business in the State of California for the entirelength of the contract. The questions submitted, andthe District’s responses, shall be posted on the District’s website athttp://www.cosumnescsd.gov.Proposal SubmittalProposals must be submitted no later than 'HFHPber , 2025 at 10am. Proposals receivedafter this time and date will not be considered. Proposers must submit an electronic copy (asingle .pdf file containing all submitted material) to the RFP Coordinator: Kavita Takhar (Email:KavitaTakhar@cosumnescsd.gov). Any submission to District by email, will receive a reply thatthe proposal has been received (during regular business hours). If a reply is not received, pleasecontact Kavita Takhar at KavitaTakhar@cosumnescsd.gov.Any Proposal received prior to the submission date and time specified may be withdrawn ormodified by written request by the firm who submitted the proposal. To be considered, however,the modified Proposal must be received by the submission date and time specified.The Contactor Proposal is the basis for negotiation of an Agreement. If agreement is reached,a written Agreement will be prepared and executed by the General Manager of the District. TheAgreement will not be executed until all required documentation has been received.This Request for Proposal does not commit the District to enter into an Agreement, to pay anycosts incurred in the preparation of a Proposal in response to this request, or to procure orcontract for services. The District reserves the right to accept or reject any or all Proposalsreceived as a result of this request, to negotiate with any qualified firm or to modify or cancel inpart or in its entirety this Request for Proposal if it is in the best interests of the District to do so.SECTION 7 – PROPOSAL SUBMISSION REQUIREMENTSThe following guidelines are provided for standardizing the preparation and submission ofproposals. The intent is to assist proposers in the preparation of their submissions and to assistthe District by simplifying the review process providing standards for comparison of submissions.Proposals submitted in response to this RFP shall include a complete response to therequirements in this section in the order presented. The District discourages lengthy and costlyproposals. Proposals should be a straightforward delineation of the proposer’s capability to4
