The following clarifications, changes, additions, deletions or corrections shall become a part of the bid

package for the project named on the previous page and all other conditions shall remain the same.

Prospective bidders shall acknowledge receipt of this Addendum No. 1 in the space provided on the

Acknowledgement of Addenda form of the bid package by the number of this addendum.

Item No. 1: Bid section “Checklist of Required Documents” (Q1, Q2) is modified with the deletion of the

checkmarks for the following Checklist items:

• Prevailing Wage Certification of Compliance

• Contractor Listing (as applicable)

• Responsible Bidder Certification

• Responsible Bidder Checklist

• Experience, Performance, and Financial Capability Questions

• Equipment Certification

• Public Works Contractor Certification(s) (contractors and

subcontractors)

• Security Deposit in the amount of 1 year’s lease

• Payment Bond

• Performance Bond (see exhibits)

• Affirmative Action Certificate, AA302 or letter of approval

• Initial Project Workforce Report (AA201)

• Subcontractors: Certificate of Insurance for the length of the Project

naming the Borough of Lincoln Park as an additional insured for the

“Cell Tower Co-Locator Project”

• Final List of Subcontractors

• Final Certified Payroll

• Any other documentation required under the Scope of Work

For clarification, it has been determined that these documents are not required for this bid. Pages Q1

and Q2 in the bid package are to be replaced in their entirety with pages Q1 and Q2 found below.
