It is recommended that responders to this RFQ check this pageprior to making their submittal.Funding for the project is from the State and Federal. The Division of Public Works (DPW) will administerthe project according to the terms and conditions of State laws and guidelines. The Construction Testing firmwill receive general instructions through the State. A Project Manager from DPW will be assigned to serve asproject manager and liaison between the Department of Administration, Idaho Division of Veteran Services,and the Construction Testing firm.The Construction Testing firm shall warrant the following: not knowingly hire or engage any illegal aliens orpersons not authorized to work in the United States as required by Title 67, Chapter 79, Idaho Code. ThePage 1“Providing responsive, cost effective, and timely support services to Idaho’s policy makers, public agencies, and state agencies as they serve Idaho citizens.”Construction Testing firm shall take steps to verify that it does not hire or engage any illegal aliens or personsnot authorized to work in the United States; and that any misrepresentation in this regard or any employmentof persons not authorized to work in the United States constitutes a material breach and shall be cause for theimposition of monetary penalties and/or termination of any Contract resulting from this RFQ.DESCRIPTION OF PROJECTThis project is an expansion and remodel of the existing Lewiston Veterans Home facility. The current facilityconsists of 66 residents in double occupancy resident rooms with a shared restroom for every two rooms. The currentdesign is to expand and remodel the home to convert all rooms into single-occupancy rooms, each with a single,private restroom. The design expands the existing facility to the north to create two new household wings with atotal of 33 new single-occupancy bedrooms and remodels the existing three resident wings to convert the existinghousehold wings into the other 33 single-occupancy rooms.This project is planned to be a four-phase project. Phase 1 includes extensive sitework around the north and eastsides of the building to redo the existing fire lane and to provide the space needed for the new addition. There is anew, continuous retaining wall around the fire lane. Phase 2 is the 23,492 square foot addition to the north of theexisting 50,803 square foot building. The new structure at the expansion is primarily slab on grade, although thereis a small basement area on the east end with a concrete topped hollow-core plank slab at the main floor level. Abovefloor structure is structural steel with open web steel joists and joist girders supporting the metal roof decking. Assoon as Phase 2 is occupiable, the staff will relocate residents to the new addition and free up the existing north andeast wings for remodel in Phase 3. After the Phase 3 remodel is complete, the west wing will be freed up for remodelin Phase 4. The remodel work under Phases 3 and 4 will include select concrete slab removal and replacement toallow for rework of the below-slab plumbing as required for the new single-occupancy restrooms.The estimated construction budget for this project is $45,000,000 exclusive of fees, commissioning, contingencies,and tests. Federal Funds / Davis Bacon wages will be associated with this project and likely Build America BuyAmerica. The construction start date is dependent on federal grant opportunity with the earliest plausible grantopportunity allowing for a schedule to commence on or about March 2027 and completion by November 2029.Construction is contingent on federal grant opportunity being awarded.REQUIRED SERVICESThe State is requesting qualification-based submittals for a Construction Testing firm starting immediately andthroughout construction administration.The Construction Testing firm is required to have all licenses and certifications for testing and inspection ofmaterials per Project Technical Experience described in this RFQ.The Construction Testing firm will be required to upload all testing and inspection reports to DPW’s web-based construction management software.The Construction Testing firm will be required to provide comprehensive general liability insurance, worker’scompensation, as well as professional liability insurance.The Construction Testing firm will represent the Owner, the Division of Public Works (DPW), reportingdirectly to the assigned DPW Project Manager/Owner’s Representative and will coordinate with theConstruction Manager and Design Professional. The Construction Testing firm shall have the ability to respondto calls for testing within 24 hours’ notice. Test results shall be made available within 24 hours of the testperformed.Page 2“Providing responsive, cost effective, and timely support services to Idaho’s policy makers, public agencies, and state agencies as they serve Idaho citizens.”REQUIRED PROJECT TECHNICAL EXPERIENCE/REQUIREMENTSThe Construction Testing firm may provide services including, but not limited to testing and inspections for, soilscompaction, aggregates, retaining walls, cement, concrete formwork, concrete mixes, structural steel, includingshop fabrication, welding, bolt connections, reinforcing steel, post-installed and cast-in-place anchors, concretemasonry units, erection of precast concrete (hollow-core), seismic systems, cold-formed steel framing, asphalt andconcrete paving, concrete sidewalks, fire-rated penetrations, and protection during cold weather.QUALIFICATION SUBMITTAL CONTENTA. Basic Qualifications: (Max 20 Points)1. Provide basic data relative to the Construction Testing firm’s company size, history, personnel,certification and experience for the Project’s Technical Requirements, resources available to meet theproject schedule and duration, and general information. Individual resumes, awards, associations, etc.,may be included. (Max 8 points)2. Provide information (at least three projects within the last five years) that validates that the companyhas experience and technical certification for the Project’s technical requirements. (Max 8 points)3. Provide information if the Construction Testing firm uses any software programs to monitor, track, andassign tasks. (Max 4 points)The Division of Public Works reserves the right to investigate and confirm the proposer's financial responsibility.This may include financial statements, bank references and interviews with past consultants, employees, andcreditors. Unfavorable responses to these investigations are grounds for rejection of proposal.B. Team Member Qualification: (Max 36 Points)1.List the individuals expected to accomplish the work. Describe who will perform the varioustasks, the amount of their involvement and responsibilities, their qualification, and relevantexperience to the project’s technical requirements. (Max 12 points)2.List three projects each individual has completed construction testing and/or inspectionswithin the last five years. Name the project, the Owner, cost of construction, and what weretests/inspection completed by the individual. (Max 12 points)3.Describe the Construction Testing firm and individual’s capability and availability to perform allof the testing/inspections during the duration of the project/work. (Max 5 points)4.The work required must be conducted by a qualified testing firm/individual who is approved by aqualified testing and inspecting agency meeting the requirements of ASTM and shall be certifiedby the Building Official. The technical project requirements also require a licensed GeotechnicalEngineer to perform some of the project inspections. Provide Idaho certifications and licenses forthe individuals performing the work. (Max 7 points)C. Examples of Work: (Max 20 Points) Provide FOUR examples of previous projects for each individualperforming the construction testing/inspections that are the same testing/inspection requirements required for thisproject. Examples include the date of the testing/inspections, project name, location, project owner, square footage,project construction date, and a reference/contact for each project.Page 3“Providing responsive, cost effective, and timely support services to Idaho’s policy makers, public agencies, and state agencies as they serve Idaho citizens.”D. Format: (Max 4 Points) To assist evaluation it is desirable to format the proposal similar to the headingslisted above. The proposals should be clear and to the point. Primary contact should be clearly identified on thecover sheet with an email address.SUBMITTALSubmit five (5) copies of the submittal; include one USB drive containing a PDF of the submittal. In your RFQcover letter, include the email address of the primary contact person; failure to provide this information mayresult in the proposal being nonresponsive.EVALUATIONA selection committee consisting of two (2) persons from DPW, two (2) persons from Idaho Division ofVeterans Services, and an independent selection committee member will evaluate and rank the firm’ssubmittals deemed to be the most highly qualified to perform the required services. The Selection Committeemay choose to interview any, all or none of the respondents as may be in the best interest of the State. The Construction Testing firm invited for an interview will receive a final scorebased on the sum of the Selection Committee’s initial ranking and final ranking.The timeframe for the firms invited for an interview is approximately 45 minutes: 25-30 minutes for the presentation;15-20 minutes for the Selection Committee’s Q&A; and 5 minutes for the Construction Testing firm’s closingcomments.AWARDBased on the results of the final rankings, DPW will recommend a course of action to the PBFAC at their nextregularly scheduled meeting. Upon selection of a firm, the State will issue a letter of intent. However, final award is contingentupon the successful negotiation of an Agreement.The contents of the submittal may be used in a legal contract or agreement. Proposers should be aware thatmethods and procedures proposed could become contractual obligations. The State reserves the right to rejectany or all proposals received as a result of this request.The State may also negotiate separately with any source in any manner necessary to serve the best interests ofthe State of Idaho. Awards will be made on the basis of submittals resulting from this request and subsequentinterviews.ATTACHMENTContractor’s draft schedule to be used for general durations and sequences.
