The District reserves the right to re-negotiate or cancel this contract prior to any renewal terms.3 Applicable SpecificationsProposals should duplicate our specifications as near as possible. All items on the Specifications Questionnairemust be answered. Do not answer any question by responding with "see brochure/attachment." This will cause yourproposal to be rejected.The policy issued by the selected company will provide coverage that duplicates the answers on the SpecificationQuestionnaire. In the event of any variation, the company or completing agent warrants the policy will be amendedto comply with the District specifications or they (the company or agent) will be responsible for providing benefits as described inthe answers to specification questionnaire.The company awarded the contract for the UIL/Athletic insurance will also receive the contract for the StudentVoluntary Insurance. The company awarded the contract for the UIL Athletic Insurance may differ from thecatastrophic coverage. Please state if your proposal is an "all or none" proposal.The catastrophic insurance carrier and basic insurance carrier must have an A.M. Best rating of at least A-(VII).(Required: Maximum 4000 characters allowed)Page 3 of 10 pagesDeadline: 3/30/2026 02:00 PM (CT)2026-1124-RFP4 Requested Coverage(Individual Voluntary and Blanket All Interscholastic Activities Coverage)A. 24-Hour- Up to $30,000, $500 deductible, for injuries sustained during the year. The coverage will be aPRIMARY POLICY. Specify if plan is accepted on FULL ASSIGNMENT through a PPO or special network group.(See Section I, PROPOSAL SUBMITTAL)B. At School - Up to $30,000, $500 deductible for injuries that occur at school or school sponsored activities and to and from school,includes all sports, except varsity football. Plan will also be primary. Specify if plan is accepted on FULL ASSIGNMENT through a PPOor special network group. (See Section II, PROPOSAL SUBMITTAL)C. Voluntary Football Coverage Varsity - Up to $30,000 for injuries sustained while practicing or participating ininterscholastic varsity football. Benefit coverage will match requirements as outlined in the at-school plan. (SeeSection III, PROPOSAL SUBMITTAL)D. Dental - Pays Usual and Customary to $1,000 for injuries to sound teeth. (See Section IV, PROPOSALSUBMITTAL)E. Catastrophic - Lubbock Independent School District will purchase blanket catastrophic accident insurance for those studentsparticipating in all UIL sports and activities. There will be a $25,000 deductible with indemnity amount of no less than $1,000,000.Deductible period will be two (2) years and a ten (10) year minimum benefit period. (See Section V, PROPOSAL SUBMITTAL)F. Blanket Coverage-All Interscholastic Activities Coverage - A $30,000 maximum policy with $500 deductible forinjuries sustained while practicing or participating in any UIL event. The blanket policy will cover all UIL activitiesgrades 6-12 and a list of names will not be provided. The policy will be excess and have an effective date of August1, 2026 and expires July 31, 2027. Benefits must be accepted on full assignment through a PPO or special networkgroup. Policy coverage must include all Vocational classes, ROTC, and FFA. (See Section VI, PROPOSALSUBMITTAL)G. Please show process necessary to maintain HIPAA and FERPA compliance. (See Section VII, PROPOSAL SUBMITTAL)H. Award will notnecessarily be made to the respondent submitting the lowest priced offer. Solicitations will be evaluated/awardedusing the best value method as defined in Texas Education Code 44.031. In determining best value the Districtshall consider the following factors:50 Points - Purchase Price20 Points - Reputation10 Points - Quality10 Points - Meets District's Needs5 Points - Past Performance0 Points - HUB Status5 Points - Long-Term Cost0 Points - Parent Company/Majority Owner in TX0 Points - Other Relevant Factors8 Fixed-Price (Price Adjustment Allowance)1. Prices shall be firm for the specified contract term.2. Prices can be adjusted based on clearly identified escalation provisions3. The District reserves the right to evaluate, accept, or reject the applicability of any price adjustment9 Non-Collusion StatementBy submitting a response, the supplier certifies and represents to the District that:a) This response has been arrived at independently and prepared without consultation, communication, oragreement with any other supplier, and that the contents of this response have not been communicated by thesupplier, or supplier agent, to any other supplier engaged in this type of business prior to the submission deadlinefor the purpose of restricting competition.b) This response is not the result of, or affected by an unlawful act of, collusion with another company engagedin the same line of business or commerce, or any act of fraud punishable under current local, state, and/or federalordinances, statutes, regulations, and/or policies.
