Allwork shall be done in accordance with the latest building codes, state and federal laws relative tothis project.INSURANCEThe attached Insurance Checklist (which includes a section for the Insurance agent to fill-out) andGeneral Contract Form shall be completed and returned with the quote package. Successful vendorshall provide certificate of insurance, as specified, prior to contract release by Purchasing.DRUG FREE WORKPLACEAll Contractors must execute the enclosed Drug Free Workplace Affidavit to verify compliancewith TCA 50-9-113 and return same with quote response. Failure to comply with thisrequirement will declare that quote non-responsive.SECTION I – PURPOSE OF THE DRUG & ALCOHOL TESTING PROGRAMThe City of Johnson City recognizes its responsibility to provide safe and efficient operations forour employees, our citizens and the general public.Our commitment to provide safe andefficient operations is shown by the implementation of programs and procedures which ensurecompliance with appropriate safety measures, as well as the letter and intent of all applicable lawsand regulations. There is sufficient evidence to conclude that the use of illegal drug/alcohol;drug/alcohol dependenceand drug/alcohol abuse seriously impairs an employee’s performance and general physical andmental health. The illegal possession and use of drugs, alcohol and/or narcotics by employees ofthe City is a crime in this jurisdiction and is clearly unacceptable. Therefore, the City of JohnsonCity has adopted this written policy to ensure an employee’s fitness for duty as a condition ofemployment; to ensure the drug tests and alcohol tests are conducted on safety-sensitivepositions in the categories of: pre-employment, randomtesting, suspicion testing, and return-to-duty testing.To comply with TCA Title 50 Chapter 9 Part 1, all bidders and/or those proposing to do servicewith the City must have a testing program of the same or better than the requirements of the City ofJohnson City.STATE CONTRACTORS LICENSE (IF QUOTE IS $25,000+)Contractor must be a licensed contractor in the State of Tennessee, as required by the Contractor'sLicensing Act of 1994, State of Tennessee. Work shall be performed during normal school hours unless otherwiseapproved by Johnson City Schools.1.3 Site Condi�onsThe Contractor shall examine the work site prior to submi�ng a bid and shall beresponsible for verifying all dimensions and condi�ons affec�ng the work.2. Materials2.1 Carpet Tiles• Product: Kinetex Carpet Tiles• Color/Patern: TBD• Size: 24" by 24"• Manufacturer: Kinetex• Warranty: 10-year commercial warranty2.2 Toe Boards (Cove Base)• Material: Commercial-grade vinyl or rubber• Height: 4 inches• Color: TBD• Warranty: 10-year commercial warranty2.3 Adhesives and Installa�on Materials\• All adhesives and installa�on materials shall be compa�ble with Kinetex carpet �lesand approved by the manufacturer.• Adhesives shall be low-VOC and meet all applicable environmental standards.3 Workmanship3.1 Installa�on• Carpet �les shall be installed in accordance with the manufacturer's instruc�ons andindustry best prac�ces.• Seams shall be �ght and inconspicuous.• Tiles shall be installed with consistent patern alignment and direc�on.• Toe boards shall be installed with �ght joints and securely fastened.3.2 Furniture Handling• The Contractor shall move all furniture and equipment as necessary to perform thework.• Furniture shall be protected from damage during the work.• Upon comple�on, furniture shall be returned to its original posi�on.3.3 Site Cleanup• The Contractor shall maintain a clean work area and remove all debris and wastematerials daily.• Upon comple�on, the work area shall be le� in a clean and orderly condi�on.4 Warranty4.1 Materials Warranty• The Contractor shall provide a warranty for all materials supplied, covering defects inmaterials and workmanship for a period of 10 years from the date of acceptance byJohnson City Schools.4.2 Workmanship Warranty• The Contractor shall provide a warranty for workmanship, covering defects ininstalla�on and performance for a period of 10 years from the date of acceptance byJohnson City Schools.5 Submitals5.1 Product Data• The Contractor shall submit product data for all materials proposed for use, includingmanufacturer's specifica�ons, warranty informa�on, and installa�on instruc�ons.5.2 Samples• The Contractor shall provide samples of carpet �les and toe boards for approval byJohnson City Schools prior to ordering materials.5.3 Insurance and Licenses• The Contractor shall provide proof of insurance and all necessary licenses as requiredby Johnson City Schools.
