Summary

The City of Rock Hill, South Carolina ("City") is seeking proposals from interested and qualified companies to provide the City with contracted On-Site Health and Wellness Services.

Background

The City of Rock Hill On-Site Health and Wellness Center provides primary health care, acute and episodic health care, wellness services, health screenings, identification and outreach to patients exhibiting markers for certain chronic health conditions in the City’s population, outpatient occupational healthcare, and other services depending on the needs of the City. The On-Site Health and Wellness Center is a health care option for City employees, retirees, and their dependents covered by City health insurance. The Center is located at the City Operations Center, 757 Anderson Road, Rock Hill, SC, with some scheduled service times available at other City facility locations.
