GENERAL INFORMATION
1. THE CONTRACTOR SHALL CONDUCT ITS OPERATIONS WITHIN THE WORK LIMITS INDICATED ON THE DRAWINGS FOR THE
PROJECT SITE, EXCEPT WHERE SPECIFICALLY NOTED OTHERWISE ON THE DRAWINGS.
2. THE GENERAL SEQUENCE OF WORK WILL INCLUDE THE FOLLOWING:
a. JONES FLOOD GATE: REMOVAL OF SELECTED PORTIONS OF EXISTING ASPHALT PAVEMENT AND PORTIONS OF THE
CONCRETE FLOOD GATE SILL, STABILIZATION OF PAVEMENT SUBGRADE, AND INSTALLATION OF REINFORCED
CONCRETE PAVEMENT AND SELECTED CONCRETE FLOOD GATE SILL REPLACEMENT. LASTLY, ALL DISTURBED AND
SOIL FILL AREAS SHALL BE GRADED, FERTILIZED, SEEDED, MULCHED AND NETTED.
3. THE CONTRACTOR IS CAUTIONED THAT THE PROJECT SITE IS SUBJECT TO FLOODING DURING RAINFALL EVENTS. THE
FRANKFORT DEPARTMENT OF PUBLIC WORKS SHALL REVIEW FORMAL REQUESTS FROM THE CONTRACTOR FOR TIME
EXTENSIONS IF A FLOOD EVENT AFFECTS SITE ACCESS TO INCREASE THE PROJECT SCHEDULE BASED ON THE REAL
IMPACTS TO THE PROJECT CRITICAL PATH BASED ON THE CONTRACTOR'S MOST RECENT SCHEDULE. THE FRANKFORT
DEPARTMENT OF PUBLIC WORKS SHALL NOT CONSIDER ANY EQUITABLE ADJUSTMENTS TO THE CONTRACT AMOUNT
FOR TEMPORARY SHUTDOWNS, EQUIPMENT DEMOBILIZATION AND RE-MOBILIZATION, OR SITE DAMAGE ASSOCIATED
WITH RAINFALL EVENTS AND/OR HIGH WATER CONDITIONS. THE CONTRACTOR IS NOT ELIGIBLE FOR ADDITIONAL
COMPENSATION DUE TO DELAYS OR IMPACTS RELATED TO RAINFALL EVENTS AND/OR HIGH WATER CONDITIONS.
4. THE CONTRACTOR SHALL SUBMIT A WORK PLAN DETAILING THE PROPOSED SEQUENCE OF WORK AND THE
APPROPRIATE EQUIPMENT REQUIRED TO PERFORM THE WORK. THE WORK PLAN MUST BE ACCEPTED BY THE ENGINEER
PRIOR TO PERFORMING ANY OF THE WORK.
5. THE CONTRACTOR SHALL SUBMIT A PROPOSED CONSTRUCTION SCHEDULE TO THE ENGINEER PRIOR TO BEGINNING
CONSTRUCTION.
6. CONTRACTOR SHALL BE RESPONSIBLE FOR ESTABLISHING STORAGE AND STAGING AREAS FOR EQUIPMENT AND
MATERIALS OUTSIDE THE CONTRACTOR'S WORK LIMITS (IF NECESSARY). THE CONTRACTOR SHALL ESTABLISH
APPROPRIATE AGREEMENTS WITH THE OWNERS OF SUCH SITES. THE CONTRACTOR IS SOLELY RESPONSIBLE FOR
SECURING ANY APPLICABLE PERMITS FROM REGULATORS FOR WORK OUTSIDE OF THE WORK LIMITS. OFFSITE AREAS
SHALL BE MAINTAINED IN COMPLIANCE WITH THE PROVISIONS ESTABLISHED FOR AREAS INSIDE THE CONTRACTOR'S
WORK LIMITS AT THE PROJECT SITES. THE OWNER AND ENGINEER SHALL HAVE ACCESS TO ALL OFFSITE STORAGE AND
STAGING AREAS USED IN THE EXECUTION OF THIS WORK.
7. CONSTRUCTION ACTIVITIES SHALL COMPLY WITH CURRENT OSHA AND ALL OTHER APPLICABLE REGULATIONS FOR
HEALTH AND SAFETY. THE CONTRACTOR IS SOLELY RESPONSIBLE FOR PROJECT SITE SAFETY THROUGHOUT
CONSTRUCTION.
8. CONSTRUCTION ACTIVITIES SHALL COMPLY WITH ALL CURRENT AND APPLICABLE FEDERAL, STATE, AND LOCAL
REGULATIONS GOVERNING DISCHARGES INTO AND/OR UPON AIR, WATER, OR LAND. THE CONTRACTOR IS SOLELY
RESPONSIBLE FOR COMPLYING WITH THESE REGULATIONS.
PERMITTING
1. THE OWNER WILL SECURE THE FOLLOWING PERMITS FOR THE FLOOD GATE PROJECT THROUGH THE KENTUCKY
DIVISION OF WATER AND U.S. ARMY CORPS OF ENGINEERS.
- FLOODPLAIN PERMIT/STREAM CONSTRUCTION PERMIT FOR CONSTRUCTION IN OR ALONG A STREAM
- CWA SECTION 408
THE CONTRACTOR SHALL COMPLY WITH ALL CONDITIONS IN THESE PERMITS AND IS RESPONSIBLE FOR SECURING ANY
ADDITIONAL PERMITS REQUIRED PRIOR TO CONSTRUCTION.
2. THE CONTRACTOR SHALL FILE A KPDES NOTICE OF INTENT (NOI) PRIOR TO THE START OF CONSTRUCTION, AND A
NOTICE OF TERMINATION (NOT) FOLLOWING CONSTRUCTION COMPLETION WITH THE KENTUCKY DIVISION OF WATER,
AND SHALL MEET ALL REQUIREMENTS OF THESE NOTICES.
3. IN ACCORDANCE WITH ALL APPLICABLE LAWS AND ORDINANCES, THE CONTRACTOR SHALL IDENTIFY, SECURE, AND
PERMIT (IF NECESSARY) SUITABLE OFF-SITE DISPOSAL SITE(S) FOR EXCAVATED OR DEMOLISHED MATERIALS. DISPOSAL
SITE SELECTION AND ACCEPTANCE MAY REQUIRE COORDINATION WITH PERMIT REGULATORS. SHOULD THIS
COORDINATION BE NECESSARY, THE OWNER WILL NOT CONSIDER ANY CONTRACTOR COSTS OR SCHEDULE CLAIMS
WHATSOEVER ASSOCIATED WITH DELAYS OR ACTIONS REQUIRED BY PERMIT REGULATORS. REGULATORY AGENCY
COORDINATION SHALL BE THE SOLE RESPONSIBILITY OF THE CONTRACTOR.
SITE CONDITIONS
1. THE CONTRACTOR SHALL BECOME THOROUGHLY FAMILIAR WITH THE PROJECT AREA AND SITE CONDITIONS PRIOR TO
BID SUBMISSION.
2. HORIZONTAL COORDINATES SHOWN ON THE DRAWINGS ARE BASED ON THE KENTUCKY STATE PLANE COORDINATE
SYSTEM, NORTH ZONE, NORTH AMERICAN DATUM OF 1983 (NAD83).
3. ELEVATIONS ON THE DRAWINGS PRODUCED BY STANTEC CONSULTING SERVICES, INC. ARE REFERENCED TO THE
NORTH AMERICAN VERTICAL DATUM OF 1988 (NAVD88), GEOID 18.
4. EXISTING SITE FEATURES, DIMENSIONS, AND TOPOGRAPHY WERE OBTAINED FROM A SURVEY PERFORMED BY QK4, INC.
ON JUNE 16, 2025. EXISTING CONDITIONS MAY HAVE CHANGED SINCE THAT TIME. THE CONTRACTOR IS SOLELY
RESPONSIBLE FOR VERIFICATION OF ALL EXISTING SITE FEATURES, DIMENSIONS, AND ELEVATIONS PRIOR TO BID
SUBMITTAL.
5. DIMENSIONS OF THE EXISTING STRUCTURES SHOWN ON THESE PLANS ARE FOR GENERAL REFERENCE ONLY. THEY
HAVE BEEN TAKEN FROM ORIGINAL CONSTRUCTION DRAWINGS, SUBSEQUENT REHABILITATION DRAWINGS, OR
INCOMPLETE FIELD SURVEYS AND ARE NOT GUARANTEED. THE CONTRACTOR SHALL TAKE ALL FIELD MEASUREMENTS
AS NECESSARY TO ENSURE PROPER FIT OF THE FINISHED WORK, AND THE CONTRACTOR SHALL ASSUME FULL
RESPONSIBILITY FOR THE ACCURACY OF SUCH MEASUREMENTS. IF THE FIELD CONDITIONS AND DIMENSIONS DIFFER
FROM THOSE SHOWN ON THE DRAWINGS, THE CONTRACTOR SHALL USE THE FIELD CONDITIONS AND DIMENSIONS AND
MAKE THE APPROPRIATE CHANGES TO THOSE SHOWN ON THE DRAWINGS, AS ACCEPTED BY THE ENGINEER. WHEN
SHOP DRAWINGS BASED ON FIELD MEASUREMENTS ARE SUBMITTED FOR ACCEPTANCE, THE FIELD MEASUREMENTS
SHALL BE INDICATED ON THE SUBMITTED SHOP DRAWINGS FOR THE REVIEWER'S REFERENCE.
6. SOME MECHANICAL COMPONENTS AND UTILITIES HAVE NOT BEEN LOCATED AND ARE NOT DEPICTED ON THE
DRAWINGS. IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO LOCATE AND VERIFY ALL MECHANICAL COMPONENTS
AND UTILITIES, MAKE APPROPRIATE ARRANGEMENTS REGARDING RELOCATION, MAINTAIN UTILITY SERVICE
THROUGHOUT THE CONSTRUCTION PERIOD, AND MAKE FINAL RELOCATIONS AT THE COMPLETION OF WORK. THE
CONTRACTOR SHALL BE RESPONSIBLE FOR ANY DAMAGE TO THE EXISTING UTILITIES, WHETHER SHOWN OR NOT
SHOWN ON THE DRAWINGS, DUE TO ITS OPERATIONS. WHERE UTILITIES ARE SHOWN OR INDICATED ON THE DRAWINGS,
THE INFORMATION GIVEN IS IN ACCORDANCE WITH THE BEST INFORMATION IN THE POSSESSION OF THE OWNER BUT IS
APPROXIMATE. THE DATA IS NOT WARRANTED TO BE EITHER COMPLETE OR CORRECT, AND THE CONTRACTOR SHALL
ASSUME ALL RISKS RESULTING FROM CONDITIONS ARISING FROM THE APPROXIMATIONS SHOWN.
7. IT SHALL BE DISTINCTLY UNDERSTOOD THAT ANY REFERENCE TO ROCK, SOIL, OR ANY OTHER MATERIAL ON THE
DRAWINGS WHETHER IN NUMBERS, WORDS, LETTERS, OR LINES, IS SOLELY FOR THE OWNER'S INFORMATION AND IS
NOT TO BE TAKEN AS AN INDICATION OF CLASSIFIED EXCAVATION OR THE QUANTITY OF ANY OF THE VARIOUS
MATERIALS INVOLVED. THE BIDDER MUST DRAW ITS OWN CONCLUSIONS AS TO THE CONDITIONS LIKELY TO BE
ENCOUNTERED AT THE SITE.
GENERAL NOTES
SITE ACCESS
1. ACCESS TO THE PROJECT SITE SHOULD BE EVALUATED BY THE CONTRACTOR PRIOR TO BIDDING. SECURING ANY
OFF-SITE EASEMENTS, RIGHTS-OF-WAY, ETC. TO IMPROVE ACCESS OR ALLOW LOADING/UNLOADING OF EQUIPMENT OR
MATERIALS SHALL BE THE SOLE RESPONSIBILITY OF THE CONTRACTOR, AND SHALL BE OBTAINED IN ACCORDANCE
WITH ALL APPLICABLE LAWS AND ORDINANCES.
2. THE CONTRACTOR SHALL GIVE SPECIAL ATTENTION TO THE PRESENCE OF OVERHEAD ELECTRIC POWER LINES AND
BURIED FIBER OPTIC LINES AND STORM SEWER PIPES. THE CONTRACTOR SHALL BE RESPONSIBLE FOR COORDINATING
OUTAGES AND/OR RELOCATIONS WITH THE AFFECTED UTILITY AS MAY BE NECESSARY TO SAFELY PERFORM THE WORK.
IN ADDITION, THE CONTRACTOR SHALL OBSERVE AND MAINTAIN CLEARANCE FROM ELECTRIC POWER LINES AS
REQUIRED BY THE RESPECTIVE UTILITY AND OSHA REGULATIONS.
3. THROUGHOUT THE CONSTRUCTION PERIOD, THE CONTRACTOR SHALL DEPLOY AND MAINTAIN WARNING SIGNS AT SITE
ACCESS LOCATIONS IN ACCORDANCE WITH RESPECTIVE REGULATORY CRITERIA TO DETER THE PUBLIC FROM
APPROACHING THE PROJECT SITE.
SITE SETUP AND MAINTENANCE
1. THE CONTRACTOR SHALL COORDINATE PLACEMENT OF OFFICES AND EMPLOYEE PARKING WITH THE OWNER AND
ENGINEER. NO EXISTING BUILDING SHALL BE USED WITHOUT PERMISSION FROM THE OWNER AND ENGINEER.
2. AT A MINIMUM, PARKING AREAS SHALL HAVE A GRADED SURFACE OF STONE.
3. THE CONTRACTOR IS RESPONSIBLE FOR SUPPLYING ALL UTILITIES, DRINKING WATER SUPPLIES, AND SANITARY
FACILITIES NEEDED FOR CONSTRUCTION ACTIVITIES. SUCH FACILITIES SHALL BE REMOVED UPON COMPLETION OF THE
WORK.
4. SECURITY OF THE WORK SITE AREAS IS THE SOLE RESPONSIBILITY OF THE CONTRACTOR. THE CONTRACTOR SHALL
PROVIDE ALL NECESSARY SECURITY MEASURES, WHICH MAY INCLUDE TEMPORARY FENCING OR SECURITY GUARDS, AS
DEEMED NECESSARY THROUGHOUT THE DURATION OF CONSTRUCTION. THE OWNER WILL NOT BE RESPONSIBLE FOR
DAMAGES INCURRED AS A RESULT OF VANDALISM, THEFT, ETC. THE CONTRACTOR SHALL PROVIDE CAMERA
SURVEILLANCE OF THE WORK SITE THROUGHOUT THE DURATION OF CONSTRUCTION.
5. THE CONTRACTOR SHALL MAINTAIN WORK SITES IN A CLEAN AND WORKMAN-LIKE MANNER AT ALL TIMES.
CONSTRUCTION DEBRIS, WASTE MATERIALS, RUBBISH, ETC., SHALL BE REMOVED FROM THE SITE IN A TIMELY MANNER.
AT THE COMPLETION OF CONSTRUCTION, THE CONTRACTOR SHALL RESTORE AFFECTED AREAS TO THEIR ORIGINAL
CONDITION OR BETTER.
6. THE CONTRACTOR SHALL CONTROL THE GROWTH OF WEEDS AND VEGETATION IN DISTURBED AREAS, WITHIN FENCED
AREAS INSIDE THE CONTRACTOR'S WORK LIMITS, AND WITHIN OFFSITE STORAGE OR STAGING AREAS. THE
CONTRACTOR SHALL BE RESPONSIBLE FOR MOWING THESE AREAS AS NEEDED.
7. DELIVERED MATERIALS FOR THE PURPOSE OF PERFORMING THE WORK SHALL BE TEMPORARILY STORED IN AREAS
SELECTED BY THE CONTRACTOR AND ACCEPTED BY THE OWNER AND ENGINEER. THE CONTRACTOR PARKING AND
OFFICE AREAS SHALL NOT BE USED FOR TEMPORARY STORAGE.
8. STOCKPILE AREAS FOR SOILS AND/OR SEDIMENTS (NOT INCLUDING STONE AND CONCRETE AGGREGATES) SHALL BE
GRADED TO MAINTAIN POSITIVE DRAINAGE AT ALL TIMES. THE SIDE SLOPES SHALL HAVE A 4H:1V MAXIMUM SLOPE. THE
CONTRACTOR IS SOLELY RESPONSIBLE FOR INSTALLING AND MAINTAINING MEASURES THAT CONTROL EROSION AND
THE TRANSPORT OF SEDIMENT FROM THE WORK SITE INTO ADJACENT CREEKS OR STORM SEWERS. EROSION AND
SEDIMENT CONTROL MEASURES SHALL BE IN PLACE PRIOR TO ANY SITE DISTURBANCE AND SHALL BE MAINTAINED
THROUGHOUT THE WORK. AT A MINIMUM, EROSION, AND SEDIMENT CONTROL MEASURES SHALL INCLUDE SILT FENCING
AROUND DISTURBED AREAS. SURFACE DRAINAGE SHALL BE CONTROLLED WITH TEMPORARY DITCHES, CULVERTS, ETC.,
AS NECESSARY. ADDITIONAL MEASURES SHALL BE PERFORMED IN ACCORDANCE WITH THE MANUAL "KENTUCKY BEST
MANAGEMENT PRACTICES FOR CONSTRUCTION ACTIVITIES" AND IN COMPLIANCE WITH APPLICABLE PERMITS.
9. AT THE SITE, THE CONTRACTOR SHALL KEEP ROADS USED FOR SITE ACCESS FREE OF MUD AND DEBRIS.
10. THE CONTRACTOR SHALL MAINTAIN SURVEY CONTROL POINTS FOR THE DURATION OF THE PROJECT. THE
CONTRACTOR SHALL PROVIDE ALL CONSTRUCTION SURVEYING AND STAKING NECESSARY FOR SITE LAYOUT AND
CONSTRUCTION FROM THE CONTROL POINTS.
EXCAVATION AND GRADING
1. ALL CLEARED TOPSOIL MATERIAL SHALL BE STOCKPILED. DURING FINAL DRESSING, THE STOCKPILED TOPSOIL SHALL BE
SPREAD EVENLY ACROSS DISTURBED AREAS RECEIVING EARTHEN FILL. EXCAVATION WILL NOT BE CLASSIFIED FOR
PAYMENT PURPOSES TO DISTINGUISH BETWEEN EARTH, ROCK, OBSTRUCTIONS OR OTHER MATERIALS.
2. THE CONTRACTOR SHALL CONSTRUCT THE BACKFILLS IN THE APPROPRIATE ZONES AND WITH THE MATERIALS
REQUIRED IN THE DRAWINGS.
3. DENSE GRADED AGGREGATE (DGA) SHALL CONFORM TO SECTION 805.06 OF THE KYTC "STANDARD SPECIFICATIONS
FOR ROAD AND BRIDGE CONSTRUCTION" FOR DENSE GRADED AGGREGATE AND CRUSHED STONE BASE.
4. NO ADDITIONAL PAYMENT FOR QUANTITIES OF STONE AGGREGATE OR OTHER MATERIALS PLACED ON THE PROJECT
WILL BE CONSIDERED BY THE OWNER DUE TO SUBGRADE SETTLEMENT, TEMPORARY EXCAVATIONS, OR INTRUSION OF
STONE INTO SOFT SUBGRADE.
SITE RESTORATION
1. THROUGHOUT CONSTRUCTION, THE CONTRACTOR SHALL REPAIR DAMAGE TO AND MAINTAIN A MINIMUM OF ONE OPEN
LANE OF TRAFFIC ALONG LEWIS FERRY ROAD AT ALL TIMES. THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING ALL
NECESSARY PERMITS FOR WORK WITHIN THE ROAD RIGHT-OF-WAYS INCLUDING A KYTC-APPROVED TEMPORARY
TRAFFIC CONTROL PLAN.
2. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTENANCE OF ALL ACCESS ROADS, PARKING, AND STORAGE AREAS
USED DURING CONSTRUCTION, AND SHALL RESTORE AREAS TO THEIR ORIGINAL CONDITION OR BETTER ONCE
CONSTRUCTION IS COMPLETE.
3. ALL AREAS OF EXPOSED SOIL DISTURBED DURING CONSTRUCTION SHALL BE RESTORED AND RESEEDED. FINAL
GRADING, FERTILIZATION, SEEDING AND MULCHING OF ALL SUCH AREAS SHALL BE COMPLETED AS SOON AS PRACTICAL
AFTER COMPLETION OF THE RESPECTIVE PORTIONS OF THE PROJECT. ASPHALT ADHESIVE, NETTING, AND/OR OTHER
MEASURES SHALL BE APPLIED IN SUFFICIENT QUANTITIES TO SECURE THE MULCH BUT SHALL NOT INHIBIT GRASS
GROWTH. SEEDING SHALL BE PERFORMED AT THE FOLLOWING RATES:

4. AT THE COMPLETION OF WORK, THE CONTRACTOR SHALL SUBMIT A RECORD SET OF AS-BUILT DRAWINGS TO THE
ENGINEER. THESE AS-BUILT DRAWINGS SHALL SHOW ALL DEVIATIONS FOR THE ORIGINAL DESIGN. THESE DRAWINGS
SHALL BE MARKED IN RED INK ON A BLACK LINE SET OF DRAWINGS.
CONSTRUCTION RESTRICTIONS
1. THE WORK SHALL INCLUDE ONLY THOSE ITEMS ESTABLISHED IN THE CONTRACT, AS AGREED TO BETWEEN THE OWNER
AND THE CONTRACTOR.
2. ALL EXISTING STRUCTURES AND FEATURES ARE TO BE PRESERVED AND PROTECTED, EXCEPT WHERE SPECIFICALLY
INDICATED ON THE DRAWINGS.
3. ALL CONSTRUCTION ACTIVITIES SHALL BE EXECUTED IN A MANNER AND SEQUENCE THAT PROTECTS THE INTEGRITY OF
THE EXISTING STRUCTURES AT ALL TIMES.
4. BLASTING IS PROHIBITED.
5. NIGHTTIME OPERATIONS: IF THE CONTRACTOR PERFORMS NIGHTTIME OPERATIONS, THE WORK AREA SHALL BE
APPROPRIATELY ILLUMINATED PER THE APPLICABLE OSHA REQUIREMENTS TO ALLOW SAFE AND PROPER
OBSERVATION AND PERFORMANCE OF WORK.

CONTRACTOR SHALL BE SOLELY RESPONSIBLE FOR ANY DAMAGE AND REPAIR
TO THE EXISTING UTILITIES, WHETHER SHOWN OR NOT SHOWN ON THE
DRAWINGS DUE TO ITS OPERATIONS. IT IS THE CONTRACTOR'S SOLE
RESPONSIBILITY TO MAKE APPROPRIATE ARRANGEMENTS REGARDING
RELOCATION AND MAINTENANCE OF UTILITY SERVICE THROUGHOUT
CONSTRUCTION. CONTRACTOR SHALL ALSO BE RESPONSIBLE FOR FINAL
RELOCATIONS, IF NECESSARY, AT THE COMPLETION OF THE WORK.
4. THE CONTRACTOR IS SOLELY RESPONSIBLE FOR INSTALLING AND
MAINTAINING MEASURES THAT CONTROL EROSION AND THE TRANSPORT OF
SEDIMENT INTO THE STORM SYSTEM. EROSION AND SEDIMENT CONTROL
MEASURES SHALL BE IN PLACE PRIOR TO ANY SITE DISTURBANCE AND SHALL
BE MAINTAINED THROUGHOUT THE WORK IN ACCORDANCE WITH THESE
DRAWINGS. AT A MINIMUM, EROSION AND SEDIMENT CONTROL MEASURES
SHALL INCLUDE SILT FENCE AND SILT TRAPS. AS NECESSARY, ADDITIONAL
MEASURES SHALL BE PERFORMED IN ACCORDANCE WITH THE MANUAL
"KENTUCKY BEST MANAGEMENT PRACTICES FOR CONSTRUCTION ACTIVITIES"
AND IN COMPLIANCE WITH APPLICABLE PERMITS. THE EROSION AND
SEDIMENT CONTROL PLAN PROVIDED ON THIS PLAN IS FOR GENERAL
REFERENCE ONLY. THE CONTRACTOR SHALL SUBMIT AN EROSION AND
SEDIMENT CONTROL PLAN FOR ENGINEER ACCEPTANCE.
5. THE CONTRACTOR IS RESPONSIBLE FOR COORDINATING LAYDOWN AREAS
WITH THE CITY OF FRANKFORT, AS NEEDED.
