Proposals must be received at the Purchasing Office beforethe date and time specified. Proposals are to be made less FederalExcise Tax and no charge for handling unless required by law.Proposals will be made available to the public after the time ofcontract. Results will be given by mail only if requested in writingand accompanied by a self addressed, stamped business sizeenvelope.SPECIFICATIONS. Vendors must submit on items as specified. Anychanges in specifications must be noted. Vendors shall be notified inwriting if any changes to the specifications are made.AWARD. The award will be made to the responsible vendorsubmitting a conforming proposal meeting specifications at thelowest cost unless other criteria are noted in the Request forProposals. Unless otherwise noted, the award may be made byindividual items. If there is a discrepancy between the unit price andthe extension, the unit price will prevail. Discounts will not beconsidered in making award but may be offered on the invoice forearlier payment and will be applicable on the date of completion ofdelivery or receipt of invoice, whichever is later.ASSIGNMENT PROVISION. The responding vendor hereby agrees toassign all causes of action that it may acquire under the antitrustlaws of New Hampshire and the United States as the result ofconspiracies, combinations, or contracts in restraint of trade whichmaterially affect the price of goods or services obtained by the cityunder this contract if so requested by the City of Laconia.CITY’S OPTIONS. The City of Laconia reserves the right to reject oraccept all or any part of any proposal, to determine what constitutesa conforming proposal, to award solely as it deems to be in the bestinterest of the City, and to waive irregularities that it considers notmaterial to the process.PUBLIC INFORMATION. The responding vendor herebyacknowledges that all information relating to this proposal and anyresulting order (including but not limited to fees, contracts,agreements, and prices) are subject to the laws of the State of NewHampshire regarding public information.PERSONAL LIABILITY. The responding vendor agrees that in thepreparation of this proposal or the execution of any resultingcontract or order, representatives of the City of Laconia shall incurno liability of any kind.PROOF OF COMPLIANCE. The responding vendor may be required tosupply proof of compliance with bid/proposal specifications. Whenrequested, the responding vendor must immediately supply thePurchasing Office with certified test results or certificates ofcompliance. Where none are available, the City of Laconia mayrequire independent laboratory testing. All costs for such testing,certified test results or certificates of compliance shall be theresponsibility of the responding vendor.FORM OF CONTRACT. The terms and conditions set forth in anyadditional Terms and Conditions by the Purchasing Office are part ofthe proposal and will apply to any contract awarded the respondingvendor unless specific exceptions are taken and accepted and willprevail over any contrary provisions in Terms and Conditionssubmitted by the responding vendor.OFFER. The responding vendor shall also be subject to City ofLaconia contract and purchase order terms and conditions.City of LaconiaRequest for ProposalsRFP No. P25-12-02CDBG- Grant Administration ServicesThe City of Laconia, NH requests written proposals from qualified firms or individuals interested in being consideredfor hire by the City as CDBG Grant Administrator to manage a Community Development Block Grant (CDBG) Projectin Laconia.The project is a $500,000 Community Development Block Grant (Public Facility) – total project cost $1.2m – forimprovements by the Community Action program, Belknap-Merrimack (CAPBM) to the Laconia Senior Center, 184Beacon Street West., Laconia, including a commercial kitchen that would allow for on-site preparation of meals fordaily on-site congregate meals and to allow for a significant expansion of the Meals-on-Wheels program as it willallow for preparation and distribution locally rather than transporting from Concord.Specific administrative services to include the environmental review record coordination, maintenance of records,accounting, meetings, compliance with procurement and labor regulations, reporting and other federal adherencerequired by the CDBG Grant Agreement. This project will be subject to the Build America, Buy America Act, Section3, and Davis Bacon Wage Rates.This project is funded through a Community Development Block Grant from the Community Development FinanceAuthority under the provisions, and subject to the requirements, Title I of the Housing and Community DevelopmentAct of 1974, as amended.Community Development Block Grants from the NH Community Development Finance authority are under theprovisions, and subject to the requirements, Title I of the Housing and Community Development Act of 1974, asamended.
