Contractors soliciting subcontractorsshall also make a good faith effort to contract with subcontractors meeting the minority businessfirm requirements.PROJECT SCHEDULEThe project schedule will be determined by the City of Wheaton Engineering Department afterthe award.PROJECT FUNDING – DCEO GRANT (EXHIBIT L)The project will receive partial funding from a grant provided by the Illinois Department ofCommerce and Economic Opportunity. An overall Business Enterprise Program (BEP) Goal of28% has been determined with 18% of grant dollars going to minority-owned businessenterprises (MBEs or WMBEs), and 10% going to women-owned business enterprises (WBEs orWMBEs), or persons with disabilities-owned business enterprises (PBEs) for this project. This goalis based on the availability of State-certified BEP vendors to perform the anticipated directsubcontracting opportunities found in the Grant Project’s approved scope of work. This Invitationto Bid includes forms that must be completed to ensure compliance with the grant requirements.Failure to complete these forms shall render a bid or offer as non-responsive. Non-responsivebids will be rejected by the City.APPRENTICESHIP AND TRAINING CERTIFICATION FORM (EXHIBIT H)This improvement project involves the construction and maintenance of sidewalks and apedestrian overhead crossing. The City requires each bidder to submit a completedApprenticeship and Training Certification form, which is included.SCOPE OF WORKSee attached Special Provisions and drawings. In submitting abid, the Vendor agrees to enter into an agreement with the City of Wheaton utilizing the City’sstandard Construction Services Agreement.Page 2 of 4Invitation to Bid #26-12 – Scope of ServicesEXHIBIT AITB #26-12 WHEATON ROOSEVELT ROAD SIDEWALK AND PEDESTRIAN BRIDGEIMPROVEMENTSBONDS☒ Bid Bond: A Bid Bond/Bid Deposit of 10% of the full contract price is required with your bidsubmittal.☒ Payment Bond: The successful Bidder shall provide an original Payment Bond equal to onehundred percent (100%) of the full amount of the award.☒ Performance Bond: The successful Bidder shall provide an original Performance Bond equalto one hundred percent (100%) of the full amount of the award.☒ Maintenance Bond: The successful Bidder shall be required to furnish a Maintenance Bondequal to ten percent (10%) of the final contract price for the faithful performance of the Bidder’sobligation to maintain and keep in good repair, the work herein contracted to be done andperformed for one (1) year from the date of final acceptance by the City.The cost of said bonds shall be included in the total bid amount.INSURANCE REQUIREMENTS (EXHIBIT C)The Vendor must carry and maintain adequate insurance consistent with the requirements listedin Exhibit C-Insurance Requirements. Provide a current Certificate of Insurance with bid as proofof insurance.INVOICESAll invoices shall reflect the following applicable information: Agreement number, name of theProject, name of the Contractor, and the services/deliverables with the price depicted in thesame format as the offer.Invoices shall be emailed to Accounts Payable at AP@wheaton.il.us.WHEN CONTRACTORS FILE CERTIFIED PAYROLL WITH IDOL, THEY WILL RECEIVE A ‘.PDFCONFIRMATION’ COPY OF THEIR SUBMISSION. A COPY OF THIS ‘.PDF CONFIRMATION’ SHALLBE ATTACHED TO ALL INVOICES.
