Scope of Services
PRIMARY:

Provide Unit Costs for certain fiber related maintenance, installation, connections, conduit construction and repair, and fiber testing, as listed in the quote tabulation(s) below.
This project area will fall within the NSBU’s service area (Map of Fiber System will be made available to successful contractor).
The Base Quote Tabulation – Construction Related Improvement will be comprised of scheduled system improvements to conduit, pull boxes and as otherwise listed below.
The Alternate Quote Tabulation will be fiber specific connections, testing, and other technical items.
The awarded Vendor will be issued an Open Purchase Order by the COMMISSION for work throughout the year as deemed necessary by the COMMISSION. The amount of the award shall not exceed $70,000.00. The quote award may be split between two VENDORS. The term of the award shall be one year. New Smyrna Beach Utilities reserves the right to extend this contract up to three (3) additional one (1) year periods based upon mutual agreement.
Monthly/Annual Inspection Services
Maintenance Agreement Details: NSBU Has approximately 81,500ft of fiber and Approximately 90 Pull boxes throughout our system

• Monthly Inspections – Visual Inspections

• Check for Tree & vegetation Overgrowth

• Pole Attachment Verification

• ID & Recommend of Pole transfers on new pole sets

• Pole Riser Integrity Inspection

• Verify Height of Cable Slack & Sag

• Check Handhole & Lid Integrity

• Verify Marker Posts are Standing Where Applicable

• Write up report of and visible damages and recommendations to prevent further damage
Yearly Inspection Details:

• Includes all items from Monthly Inspections

• Perform Optical Time Domain Reflectometer (OTDR) testing on all dark optical fibers to check for damages

• Inspection of FTP panels to include optical termination and housing inspection

• Inspect all Splice Cases – Aerial & Underground – to check for damage

• Report for any issues found and recommended corrections for the issues
Contractor to provide a Detailed Maintenance Pricing Rates schedule with their RFQ Submittal. This pricing schedule should cover everything from Hourly rates of Personnel & Equipment along with detailed Scheduled work rates.

Written quotations must be provided for all work identified as an issue or repairs that need to be made. UCNSB would need to approve the quotation prior to Contractor proceeding with recommended repairs.

VENDOR shall provide insurance, materials, tools, labor, equipment, and supervision to perform all work necessary to complete this scope of work.
VENDOR shall submit warranties and a committed response time in days with quote submittal.
