Upon completion of construction in accordance with the approved plans and specifications, thepermittee shall submit the Certificate of Construction to the Department. Certification shall statethat the work was completed in accordance with the approved plans and specifications and shall besigned by the professional engineer or other person responsible for the work. Certification shallinclude that adequate operation and maintenance information for the approved facilities is available,on site, for use by the public water system’s personnel.2. Prior to issuance of an Operation Permit, the facilities shall be properly disinfected in accordancewith 25 PA Code, Chapter 109.711 of the Safe Drinking Water regulations and by the AmericanWater Works Association (AWWA). The facilities shall be tested for coliform bacteria inaccordance with AWWA standards. The samples shall be analyzed by a Department-accreditedlaboratory. Copies of the satisfactory microbiological test results shall be submitted with theCertificate of Construction.3. The approved pumping rate of Well B is 27 gallons per minute (gpm).4. Ion Exchange Softening:A. The maximum instantaneous flow rate through the treatment system shall not exceed 22.8 gpm.B. Salt used in the softener shall be NSF Standard 60 certified.5. TDS Removal:A. No more than 51% of the total system flow shall bypass the TDS treatment.B. Cartridge Filters:1. The maximum instantaneous flow rate through the treatment system shall not exceed 11.1gpm.2. The cartridge filter shall be replaced by an ANSI/NSF Standard 61 certified cartridge filter.C. Reverse Osmosis:1. The permittee shall collect and analyze samples for total dissolve solids (TDS)concentrations from the raw water of Well B and finished water at Entry Point No. 101 atleast weekly. The samples shall be analyzed by a Department accredited laboratory or by aproperly certified operator using an appropriate calibrated field analytical test kit. A non-certified operator may also utilize an appropriate calibrated field analytical test kit, providedthe analysis is conducted in accordance with standard operating procedures approved by theproperly certified operator. The permittee shall collect finished water samples from Entry Point No.101 once percalendar year quarter. The samples shall be analyzed by a Department accredited laboratoryfor iron and manganese and reported to the Department.D. Acid Neutralizer:1. The maximum instantaneous flow rate through the treatment system shall not exceed 11.1gpm.2. The acid neutralizer media shall be replaced by CSI NE10 pH neutralizing media only.3. The acid neutralizer media shall be replaced at least every 6 months to 12 months (per CSI’srecommendations).6. Sodium Hypochlorite Disinfection:A. The maximum instantaneous flow rate through the contact tank shall not exceed 20 gpm and theminimum water level in Tank No. 2 shall not be less than 230 gallons (9” water level) in order tomaintain a minimum of 20 minutes of disinfection contact time for Entry Point No. 101.B. Sodium Hypochlorite used in the treatment system shall be NSF Standard 60 certified.C. The Public Water System shall be monitored weekly for free chlorine at the entry point. Recordsof chlorine residual monitoring shall be kept onsite for a minimum of five years and madeavailable to the Department upon request.D. A water quality complaint log, equipment maintenance records, and chemical usage log shall bemaintained onsite. These logs and records shall include the amount (gallons per day or inches perday) and mixing ratio of sodium hypochlorite fed daily.7. An equipment maintenance log shall be maintained onsite. Records shall be kept for a minimum offive years and shall be made available to the Department upon request.8. The permittee shall retain the services of a properly certified water operator for the public watersystem. In the event permittee terminates the contract of the properly certified operator, they shallemploy a new, properly certified operator within 10 days.
