What is the method of compensation for your current consultant? Invoiced monthly7. Which employees are eligible to participate in your benefits plan? Full-time employees and pre-Medicareretirees8. What is the name of your current major medical carrier and how long have you been using them? AnthemBlue Cross/Blue Shield, 10+ years11. What is the name of your current dental carrier and how long have you been using them? Delta Dental, 10+years12. Do you currently have either an FSA or HSA in place? HSA13. Can you provide a chart showing the full monthly premium for each medical and dental plan to include abreakdown of the Employer contribution and what the Employee must pay? No.14. Does Christiansburg currently utilize an outside service to handle PPACA reporting and compliance or is thishandled internally? Internally15. Who currently administers COBRA for you? In a typical year, how many contact hours does Christiansburg spend in direct communication with your leadbenefits consultant? It depends on claim concerns, etc.17. During your annual open enrollment, how many on-site hours do you typically need from yourconsultant? None18. How many different sites generally hold meetings? 819. Please provide an estimate of the number of hours per month you anticipate needing for claim resolutionservices. It depends on the claims.20. Please provide a list of voluntary benefits offered which includes the names of the carriers for each product.Our voluntary benefits currently include accident, hospital indemnity, critical illness, short-term disability,legal aide, and optional life. We are open to considering different carriers and voluntary benefits. At thistime, we are not limiting submissions to specific carriers and welcome recommendations for the productsoffered.21.
