ObjectiveThe selected contractor shall supply vision services to the resident population of theDepartment of Human Services (“DHS”) Southeast Youth Development Center (“SYDC”),Collegeville, PA. SYDC is located at 1212 Mokychic Drive, Building 12, Collegeville, PA19426. SYDC is a youth development center for juveniles.II. BackgroundThe selected contractor shall provide vision services at SYDC, consisting of visionexaminations, eye glass frames, lenses, and repairs.III. Term of ServiceThe term of this Service shall commence upon issuance of a Purchase Order (“PO”) to theselected contractor (“Effective Date”) and shall expire three years after the Effective Dateunless it is terminated earlier pursuant to the terms of the PO.The Commonwealth shall have the option to renew the PO upon the same terms andconditions for two additional one-year terms. At the time of each renewal, the Departmentand the selected contractor shall negotiate the PO unit price not to exceed a maximum of3% over the unit price in effect at the time of each renewal term. The term of this Projectmay be extended by and at the sole option of the Commonwealth for up to 3 months uponthe same terms and conditions where a continued need exists for the services of the selectedcontractor and there has been no termination under the terms of the PO.IV. Scope of WorkAll examinations must be performed by a licensed optometrist or ophthalmologist. Theselected contractor shall provide an optometrist or ophthalmologist that will performprimary and specialty eye and vision care services and/or procedures.A prescription form for any glasses, if needed, must be completed by the licensedoptometrist or ophthalmologist at the time of the examination and given to the Nursingstaff.Page 1 of 4Revision Date: July 14, 2023The selected contractor will provide frames, lenses (single, bifocal, or trifocal) and regulartint for lens glare, and diopters SPH#4, CYL2.The selected contractor must bring their own examination equipment.The selected contractor will perform adjustments and minor repairs, defined as brokenframes, lost screws, or cracked lenses.The selected contractor will provide current styles of frames, and no wire frames, that areappropriate for facility population and are within the agreed upon price range as noted inthe bid pricing.The selected contractor shall ship the glasses as they are available.The selected contractor’s staff must sign in and out at the front entrance/clerical area.V. Specific RequirementsSYDC will provide an area suitable for performing eye examinations within the SYDCFacility. Due to safety and security, it would be ideal to have the selected contractor cometo the facility.VI. Bid RequirementsA bidder must provide the following when requested. Failure to submit these items whenthey are needed may result in the rejection of the bidder’s submission.• Proof of Liability Insurance;• Optometrist/Ophthalmologist License from the state of Pennsylvania;Completed Workers Protection and Investment Certification;VII. Questions and AnswersAll questions regarding the IFB must be submitted via the email scover@pa.gov address.All questions must be received by 06/12/26. Include the IFB bid number in the subject ofthe email.All questions and answers are considered an addendum to the IFB. Answers will be postedby 06/19/26.VIII. Payment Provisions:The selected contractor will be reimbursed only for commodities/services performed andaccepted by the Commonwealth of Pennsylvania.Failure to submit invoices in compliance with the following instructions will result in theinvoices being returned to the selected contractor and will substantially delay processingof payments. The selected contractor shall be paid upon satisfactory delivery/completionof work performed and submission of an invoice on the contractor’s letterhead.The invoice should contain at minimum the information listed on the sample invoice –Supplier Sample Invoice can be found at:http://www.dgsweb.state.pa.us/comod/CurrentForms/SampleSupplierInvoice.docXIII. Prison Rape Elimination Act RequirementsIn accordance with its zero-tolerance policy, DHS prohibits all forms of sexual abuse andsexual harassment of juvenile in its facilities. The selected contractor shall comply withDepartment policy and with the Prison Rape Elimination Act of 2003 (“PREA”) and itsregulation at 28 C.F.R. Part 115 Subpart D, Standards for Juvenile Facilities.Page 3 of 4Revision Date: July 14, 2023At a minimum, the selected contractor shall provide a copy of DHS’s Zero Tolerance forSexual Abuse and Harassment pamphlet to all staff members, including staff ofsubcontractors who have any contact with juveniles at the facility and shall provide to DHSconfirmation of each staff member’s receipt and understanding of the pamphlet as well asto any new staff that may be hired for the duration of the contract. The selected contractorstaff members and subcontractors, if any, will not be permitted to provide services untilcompleted sign-off forms are received for those individuals who may come in contact withjuveniles at the facility.DHS may require the selected contractor’s staff, including that of subcontractors, toundergo additional DHS provided training on their responsibilities under DHS’s sexualabuse and sexual harassment prevention, detection, and response policies and procedure.DHS will determine whether additional training is required based upon the nature ofcontracted services and the level of contact with juveniles at the facility.Failure to meet the DHS’s requirements regarding PREA training may result inconsequences ranging from an individual’s inability to provide services up to and includingtermination of the contract.Page 4 of 4Revision Date: July 14, 2023
