Bid Title: Non-Emergency Transportation Services
Category: Job and Family Services Status: Open Description: LEGAL NOTICE Fulton County Job & Family Services is receiving proposals for non-emergency transportation services for consumers referred from Fulton County Job and Family Services (FCJFS). This includes transporting individuals in Fulton County and the surrounding areas. FCJFS wishes to contract with an experienced transportation provider from January 1, 2026, to December 31, 2026.
The complete Request for Proposal (RFP) is available on our website at www.fultoncountyoh.com under E-Services/ Bid Postings. RFP packets may also be obtained or examined at the FCJFS office at the address below, Monday through Friday between the hours of 8:00 a.m. and 4:00 pm.

FCJFS will endeavor, but shall not be required, to answer any such request for information. All answers shall be provided to all persons/firms who have received copies of this RFP and posted online. This contract is contingent upon the availability of funding. If funding is not available for the proposed services, this contract may be terminated or reduced as determined necessary by the agency.

FCJFS is not required tocompensate the vendor for any expenses incurred as a result of the RFP process.VENDOR PROJECT REQUIREMENTSProgram DescriptionFCJFS is seeking a provider(s) for transportation services. The goal of the program is to providetransportation services for customers referred from FCJFS for agency, medical (including stretcherservice), work-related appointments, and other transports in Fulton County and the surrounding area.Program CharacteristicsThe Transportation Provider must be:1. Culturally responsive to the customers they transport.2. Respectful of the customers they transport.Staff RequirementsExperience in providing transportation services to individuals in the immediate area of Fulton County,and/or individuals of similar locations and characteristics.Indicate which staff will be involved directly with providing the transportation services.Staff resumes must be submitted with the bid.It is expected transportation services include the need for the services at various times throughout theday.Vendor RequirementsFamiliarity with Fulton County is beneficial but not required. Medical, psychological, psychiatric, therapeutic, diagnostic services or treatment.2. Services that can be paid for by third party resources such as Medicaid, private healthinsurance, etc.3. Construction or purchase of facilities or buildings.4. Purchase of equipment.Target PopulationThis proposal is limited to serving Fulton County residents who are referred for services by FCJFS.Project DeliverablesThe vendor must be able to measure and report the following statistical information during thecontract period:Service Delivery Expectations:● The number of individuals served who are transported to his or herappointments timely.Output Effectiveness:• the number of individuals served• the number of individuals who are no-calls, no-shows• the mileage• the number of tripsBilling RequirementsInvoices should be submitted via email to Fulton_Fiscal@jfs.ohio.gov on a weekly basis following theweek in which services were provided. The invoice will include the miles transported per trip, thename of individuals served, the destination transported, the negotiated rate and total rate for thetransportation provided. Invoices should include proof of signature by the customer who wastransported and the total mileage for the trip. Beginning and ending mileage for each trip should alsobe provided.SUBMITTING BIDSDelivery of Bids3Vendors must mail or deliver one signed, sealed original proposal to Fulton County Job & FamilyServices, 604 S. Shoop Avenue, Suite 200, Wauseon, Ohio 43567, no later than 4:00 p.m., December19, 2025.Acceptance and Rejection of BidsFCJFS reserves the right to reject any or all Bids.Opening of BidsThe sealed bids will be opened by FCJFS in a public meeting on December 23, 2025, at 10 :00 am atFulton County Commissioners, 152 S. Fulton St., Wauseon, Ohio, in the 2nd floor conference roomContract Period-Funding & InvoicingThe contract begins on January 1, 2026, and ends on December 31, 2026. The vendor shall claimpayment only for eligible services provided, in the amount determined by the negotiated budget.Reimbursement by FCJFS will be made within 30 days of receipt of qualified weekly invoices and anyrequired documentation or revisions.Confidentiality & SecurityAny vendor with access to confidential information will be required to keep that informationconfidential.CONTRACT COMPLIANCE REVIEW AND MONITORING POLICYThe contract requires the completion of contract compliance reviews, or monitoring, to ensure FCJFSreceives quality and cost-effective services.The scope of the review is for any given month within the vendor agreement period.Components of a Review-Financial and ServiceFinancial reviews are an examination of financial records that can include general ledger journalentries, payroll ledgers, invoices, bank statements, detailed trial balance, annual financial statement,Board minutes, etc. Actual program expenses are reviewed and compared to the program budgetsubmitted to FCJFS to determine the actual cost of the program and the appropriateness of expensesclaimed.Service reviews can include an examination of client records, rosters, quality and quantity assurancedata, observation of activities (when appropriate), and/or customer satisfaction surveys. If the vendorhad a recent audit, FCJFS requires a copy of the audit be submitted to FCJFS.Actual service documentation will be compared with services invoiced to and reimbursed by FCJFS.4A copy of the review report is given to the vendor and filed with the vendor agreement. All issuesreported must be resolved. When determined necessary by FCJFS, another review is completed withinthe vendor agreement period. This mini review is to determine and ensure adherence to thecompliance reports recommendations.Overpayment RecoveryAn overpayment occurs when there is no documentation to support service(s) invoiced and paid, theconsumer was not eligible for the service, or the vendor billed for unallowable costs or costs not in thevendor agreement budget or more than the budget line items.A repayment plan will be developed with the vendor. Except in unusual circumstances, completerepayment must be made within the vendor agreement duration unless negotiated with and agreedupon by FCJFS.BID FORMATProposal FormatTo simplify the process of evaluating bids, and to assure each proposal receives the same review, allbids must be submitted in the following format. Bids must contain all the items listed here in theformat provided. Proposal sections must be numbered corresponding to the following format:• Cover Sheet Form• Proposal Narrative• Budget• Copies of agency professional certifications and resumes• Verification of auto/liability insuranceProposal FormatThe elements listed below should be included in a detailed narrative format.• Summary: A statement as to the intent of the proposal, the basic program approach,highlighting such details as the estimated number to be served and planned outcomes.• Program Description: A detailed narration of your planned implementation and delivery ofTransportation services.• Goals/objectives and performance and effectiveness levels: Discuss target populations andproject deliverables as specified in this RFP; projected number of families to be served.• Justification for budget costs: Discuss in detail the budget line-item costs proposed and theirjustification.
