Services shall be provided for an initial term of one (1) year, with an option to renew, and shall be delivered in accordance with the objectives, assumptions, and requirements outlined in the Scope of Work.

Format and Submission of Proposals3.1 Proposal ComplianceIn order to enable direct comparison of competing responses, you must submit your Proposal inconformity with the requirements stated herein. Failure to adhere to all requirements may resultin your Proposal being disqualified as non-responsive.3.2 Proposal DeadlineAll Proposals must be received no later than the date and time specified on the Title Page, at thelocation specified on the Title Page. Proposals received after this date and time will not beconsidered. Incomplete proposals will not be considered.3.3 Proposal ContentPlease assemble your proposal in the following order:a. Cover Letter: A standard business letter must be included, which represents youragreement to supply the requested goods and/or services detailed in the RFP.b. Company Profile: Provide a brief history of your company, and an executive summary ofyour response. Content should be no more than two pages.c. Project Understanding and Approach: Provide an overview of your understanding of theneeds of the department and your understanding of the services to be provided, and yourapproach to the work.d. Scope of Work: Provide your specific proposal to address the Scope of Work outlined inSection 5 and Exhibit A below. At the very least, this should describe in detail how theservice shall be provided and include a description of major tasks and subtasks. Ifapplicable, describe administrative and fiscal management of the program, personnelpolicies, and expected use of subcontractors.e. Return Material Authorization Process: Provide an articulated Return MaterialAuthorization (RMA) process.County of Lake – Request for Proposals – Employee Wellness Servicesf. Pricing: The proposal shall outline the proposed costs of providing the service asoutlined in the Scope of Work. All prices submitted are non-binding at this stage of theprocurement process and are subject to negotiation.g. Contract: Upon acceptance of a Respondent’s proposal, County will prepare and submit acontract to the successful Respondent for signature. (See sample contract, as Exhibit B,which contains required contractual language.) If any components of the attachedcontract are not acceptable to the Respondent, the Respondent must include suchunacceptable components in their response.h. Litigation Disclosure: Disclose and explain any litigation, threatened litigation,investigation, reorganization, receivership, filing, strike, audit, corporate acquisition,unpaid judgments, or other action that could have an adverse impact on their ability toprovide the required needs.3.4 Proposal SubmissionSubmit to the appropriate location specified on the Title Page (either by mail or delivery), six (6)complete copies of your Proposal in a sealed envelope or package, clearly marked on the outside“CONFIDENTIAL” with your company name and return address, the RFP Title, and the due date.Faxed or electronic proposals will not be considered.4. Evaluation ProcessThe County is using a competitive proposal process, in which the experience and responsivenessof each submitted proposal are evaluated against the Scope of Services.Proposals shall be opened and reviewed to ensure each complies with the RFP requirements. Theabsence of required information may render the proposal non-responsive and may be cause forrejection.All proposals will be evaluated to determine whether they meet all RFP requirements.A Consultant Selection Board may be convened to review, discuss, and rank proposals using thefollowing criteria:1. Adherence to the RFP2. Experience and qualifications3. Pricing and cost effectiveness4. Reporting and monitoring capabilities5. Compliance with regulations and sustainability effortsPrior to final selection, a short list of qualified and responsive Consultants may be requested toparticipate in an interview. The purpose of the interview will be to provide an opportunity foreach Consultant to present their qualifications and proposals in person and/or to answer anyquestions that County staff may have regarding the Consultant’s submittals. If interviews are tobe held, the time and location will be arranged once the shortlist is complete.
